Does Humana Cover Home Health Care? Eligibility, Costs, and Denials

Humana does cover home health care, and for most members on a Humana Medicare Advantage plan the covered skilled services carry a $0 copay, matching what Original Medicare pays for the same care.1Humana. Home Health Services Coverage also runs through Humana’s Medicaid plans (Humana Healthy Horizons) and its Dual-Eligible Special Needs Plans, each with its own eligibility rules. What’s covered, who qualifies, and what’s left out depends on which plan you hold and what state you live in, so the Evidence of Coverage for your specific plan is the final word.

What Humana Covers at Home

Under Humana’s Medicare plans, home health coverage mirrors Original Medicare: part-time skilled nursing, physical therapy, occupational therapy, and speech-language pathology. Covered services can also include wound care, intravenous or nutrition therapy, injections, patient and caregiver education, and monitoring of unstable health conditions.1Humana. Home Health Services Medicare Advantage plans must offer everything Original Medicare does, and Humana says many of its plans add supplemental benefits on top.2Humana. What Does Medicare Cover

Humana Healthy Horizons Medicaid plans cover part-time skilled nursing, home health aide services, and skilled therapies. Authorization tiers run from three to sixteen hours of daily care depending on clinical acuity, with the top levels reserved for members who need around-the-clock monitoring for conditions like ventilator dependence or severe neurological disorders.3Humana. Who Qualifies for Humana Home Health Coverage

On the Medicare side, you need to meet several conditions at once. You must be under a doctor’s care, have a face-to-face visit with a physician or qualifying practitioner who certifies that you need the services, and require only part-time or intermittent skilled nursing or therapy. You must also be homebound, meaning leaving the house takes considerable effort and typically requires help from another person, special transportation, or a mobility device such as a wheelchair or walker.1Humana. Home Health Services Medicare defines “part-time or intermittent” as generally up to eight hours a day and twenty-eight hours a week, with short-term exceptions allowing up to thirty-five hours a week when medically necessary.4Medicare.gov. Home Health Services

Humana’s Medicaid plans don’t impose a homebound rule. The standard is medical necessity: the member’s condition has to justify care at home rather than in an outpatient clinic. That bar is met when traveling to a clinic would cause significant medical hardship or interfere with treatment effectiveness. Simple inconvenience, or a caregiver’s unwillingness to drive to appointments, doesn’t qualify.5Humana. Home Health Clinical Coverage Policy

Across both lines of coverage, services have to be delivered by a certified home health agency and documented in a plan of care signed by the ordering provider.

What You’ll Pay

Most Humana Medicare Advantage members pay nothing for covered skilled home health visits. Durable medical equipment is treated separately. Items like walkers, wheelchairs, hospital beds, and oxygen equipment ordered as part of your home health care fall under Medicare Part B, where you typically pay 20% of the Medicare-approved amount after the annual deductible.8Humana. Durable Medical Equipment A Humana Medicare Advantage plan can change that cost-sharing, so check your plan documents for the specific numbers.

What Humana Will Not Cover

This is where many families run into trouble. Medicare, and therefore Humana’s Medicare plans, does not cover 24-hour-a-day home care, and it does not cover purely custodial or personal care when that is the only kind of help you need. Custodial care means non-medical help with daily activities like bathing, dressing, toileting, eating, and preparing meals.9Humana. Does Medicare Cover Long-Term Care Meal delivery is also outside the standard Medicare benefit.2Humana. What Does Medicare Cover

Humana’s Medicaid home health policy has its own exclusion list: transportation for errands, homemaker tasks like laundry and cleaning, companion or sitter services, and respite care. Custodial services like attendant care fall under separate Medicaid waiver programs instead.3Humana. Humana Healthy Horizons Home Health Policy

Long-term care, whether at home, in assisted living, or in a nursing home, is generally not covered by any Humana Medicare or Medigap plan. Medicare Advantage may cover up to 100 days in a skilled nursing facility while you’re working toward a health goal, but once skilled care is no longer needed, coverage ends.9Humana. Supplemental Benefits That Can Fill Some Gaps

Some Humana Medicare Advantage plans offer a Personal Home Care Services benefit that covers help with bathing, grooming, dressing, using the restroom, getting in and out of bed, and preparing meals. It’s typically offered after an injury or illness, and it’s meant to give relief to family or friends who have been providing care.10Humana. Home Health and Personal Care Whether you have this benefit depends entirely on your plan and location.

Certain plans bundle additional post-discharge support. The 2026 Humana Select Partner Plan, for example, includes post-discharge personal home care with a minimum of four hours per day and up to forty-four hours per year after a hospital or skilled nursing facility stay, plus fourteen home-delivered meals over seven days after an inpatient discharge, up to four times a year, through Humana’s Well Dine meal program. Transportation benefits of up to sixty one-way trips per year are also included in some plans.11MedicareAdvantage.com. Humana Select Partner Plan (HMO) Summary of Benefits

Dual-Eligible and Medicaid Waiver Options

If you qualify for both Medicare and Medicaid, Humana’s Dual-Eligible Special Needs Plans combine medical coverage with long-term services and supports. Humana’s Fully Integrated D-SNP in Illinois, for instance, covers managed long-term services and supports that include help with everyday tasks like bathing, cooking, dressing, and taking medication, delivered at home or in a community setting. Members work with a care coordinator on an individualized plan of care, and the plan carries $0 annual out-of-pocket costs for medical services.12Humana. Medicaid waiver programs add another layer. In Virginia, the Commonwealth Coordinated Care Plus waiver covers personal care in both agency-directed and consumer-directed models, respite care, skilled private duty nursing, adult day health services, and personal emergency response systems. Waiver-enrolled members also get enhanced benefits like fall prevention kits and home-based virtual assistance technology.13Humana. Virginia LTSS Provider Resource Guide In Illinois, Humana administers five separate home and community-based services waiver programs covering elderly members, people with disabilities, individuals with HIV or AIDS, those with brain injuries, and residents of supportive living programs.14Humana. How to Get Started

The pathway to approved home health care runs through your doctor:

  • A physician, nurse practitioner, or other qualifying provider evaluates you and determines that home health services are medically necessary.
  • The provider sees you in person, either within 90 days before services start or within 30 days after.
  • The provider certifies the need and establishes a written plan of care specifying the type, frequency, and duration of visits.
  • You receive care from a Medicare-certified or plan-approved home health agency.
  • Before services begin, the agency has to tell you what Medicare or your plan will cover, what it will not, and what your anticipated out-of-pocket costs will be.1Humana. Home Health Services

Many services need prior authorization. Humana maintains a prior authorization list at Humana.com/PAL, and providers can check specific services through an online search tool or Humana’s authorization line.15Humana. Humana Provider Manual (Delegated) Under Humana’s Medicaid plans, prior authorization is required for all rehabilitation services and, for adults, all skilled nursing and home health aide services.5Humana. Once care begins, it has to be recertified. Medicare home health requires recertification every 60 days.16CGS Medicare. Home Health Certification Requirements Humana’s Medicaid policy authorizes care in 90-day blocks, with recertification required at the end of each period.3Humana. If Humana Denies Your Home Health Care

You have appeal rights. Medicare members can file an appeal within 65 calendar days of the denial notice by phone, fax, mail, or through Humana’s online portal, and an expedited appeal is available when a standard timeline could seriously jeopardize your health or ability to function. Medicaid members have a 60-day filing window, with state-specific phone numbers and mailing addresses for each Humana Healthy Horizons plan.17Humana. Appeals and Grievances

Beyond Humana’s internal process, Medicare Advantage members can get an independent review through MAXIMUS Federal, which serves as the Independent Review Entity for the Centers for Medicare and Medicaid Services. You can also request a fast-track appeal through your regional Beneficiary and Family Centered Care Quality Improvement Organization.18CMS. Managed Care Appeals and Grievances
