Does HRA Cover Copays? Plan Rules, HSA Impact, and Taxes

In most cases, an HRA does cover copays. Health Reimbursement Arrangements are employer-funded accounts built to pay back workers for qualifying medical costs, and copays sit comfortably inside the IRS definition of a qualified medical expense. The reason the answer is not a flat yes is that your employer writes the plan. They pick the annual amount, they pick which expenses are eligible, and they can choose to leave copays off the list. The only place with a definitive answer for your account is your HRA Summary Plan Document or your HR department.

Why the Plan Document Controls the Answer

An HRA is not a spending account you draw from at will. It is a reimbursement arrangement: your employer agrees to pay you back for certain medical costs up to a set annual amount. Copays, coinsurance, and deductibles are the three cost-sharing expenses HRAs are most commonly designed to cover.1UnitedHealthcare. The word doing the work there is “typically.” Some employers restrict the HRA to the deductible portion of a high-deductible health plan. The outer boundary comes from the IRS. Under Section 213(d) of the Internal Revenue Code, qualified medical expenses include the costs of diagnosis, treatment, and prevention of disease, along with payments for services from doctors, dentists, surgeons, and other practitioners.4IRS. Copay Coverage by HRA Type

Federal rules recognize several types of HRA, and the mechanics for copays differ across them.

A Group Coverage HRA, sometimes called the integrated or traditional HRA, supplements the employer’s group health plan. It can reimburse copays, coinsurance, and deductibles, but you must be enrolled in the group plan to participate. There is no federal cap on the annual contribution.6PeopleKeep. An Individual Coverage HRA reimburses employees who buy their own individual health coverage. Employers choose whether the ICHRA covers premiums only, qualified medical expenses only, or both. A Qualified Small Employer HRA is for employers with fewer than 50 employees that do not offer a group health plan. Doctor visit copays, coinsurance, and pre-deductible payments are explicitly listed as eligible medical expenses, if the employer elected to allow medical expense reimbursement in the plan terms.9Take Command Health. An Excepted Benefit HRA is a supplemental benefit that sits on top of the employer’s group health plan. It explicitly covers copays, deductibles, coinsurance, and expenses the primary plan does not cover, along with premiums for dental and vision.11CMS. How To Get Your Copay Reimbursed

The process depends on whether your employer issues a debit card or requires you to file claims.

If You Have an HRA Debit Card

Some employers issue a card linked directly to HRA funds. You swipe it at the doctor’s office or pharmacy and the copay is drawn from your balance. These cards are typically coded to work only at merchants with medical-specific category codes or at retailers with inventory systems that verify item eligibility at checkout.14Lively. Even with a debit card, the IRS requires substantiation for every transaction. Many systems auto-substantiate by matching the charge to a known copay amount. If You File a Manual Claim

Without a debit card, you pay the copay out of pocket and submit a reimbursement request online, through an app, or by mail.17Optum Bank. Many plans require the Explanation of Benefits from your insurer, especially when insurance partially paid the claim. The HSA Trap: When Copay Reimbursement Costs You HSA Eligibility

If your employer pairs an HRA with a high-deductible health plan and you also want to contribute to a Health Savings Account, the HRA’s design matters. A general-purpose HRA that reimburses copays from the first dollar disqualifies you from making HSA contributions. Two designs preserve HSA eligibility. A limited-purpose HRA restricts reimbursement to dental and vision, keeping copays off the table. A post-deductible HRA allows copay reimbursement, but only after you have met the HDHP’s minimum deductible.14Lively. Taxes on the Reimbursement

HRA reimbursements for qualified medical expenses are tax-free for employees under Section 105(b) of the Internal Revenue Code. When your employer reimburses a copay through the HRA, you owe no income tax and no payroll tax on that amount. For the exclusion to apply, the HRA must be funded entirely by the employer with no salary-reduction contributions, and each reimbursed expense must be substantiated. If the arrangement lets participants cash out unused funds for non-medical purposes, all distributions become taxable.22IRS. Notice 2002-45

What an HRA Will Not Reimburse

Even the most generous HRA cannot reimburse expenses outside Section 213(d). Common exclusions include gym memberships, fitness trackers, cosmetic surgery for non-medical reasons, teeth whitening, general vitamins and supplements, toiletries, and household items.23PeopleKeep.
