Healthy Blue does cover GLP-1 medications, but what gets approved depends on your state, your diagnosis, and the specific drug. GLP-1s prescribed for type 2 diabetes are covered across every state where Healthy Blue operates. For weight management, coverage is available to members in North Carolina and Kansas with prior authorization, and is not available to members in Louisiana or South Carolina.

GLP-1 Coverage for Type 2 Diabetes

If your prescription is for type 2 diabetes, Healthy Blue covers it in every state the plan serves. This isn’t a matter of plan policy. In Louisiana, for example, the state’s clinical criteria require a confirmed diabetes diagnosis and a hemoglobin A1C of 6.5% or higher documented within the previous six months. GLP-1 Coverage for Weight Management in North Carolina

Healthy Blue North Carolina currently covers GLP-1 medications for weight management, following a short gap in coverage. NC Medicaid first added GLP-1s for obesity on August 1, 2024, then ended that coverage effective October 1, 2025, because of a $319 million Medicaid budget shortfall.4NC DHHS Medicaid. Which Drugs Are on the List

Three medications are back on the Preferred Drug List for weight management. Wegovy (semaglutide) is the preferred product, meaning it can be approved without trying another GLP-1 first. Zepbound (tirzepatide) and Saxenda (liraglutide) are non-preferred: members must first try and fail Wegovy, or the prescriber must document a medical reason Wegovy cannot be used.

Prior Authorization Criteria

Every GLP-1 weight management prescription through Healthy Blue NC requires prior authorization. The clinical thresholds come from NC Medicaid’s outpatient pharmacy criteria:7Healthy Blue NC.   • Adults 18 and older need a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related condition such as hypertension, type 2 diabetes, obstructive sleep apnea, cardiovascular disease, or dyslipidemia.
  • Adolescents aged 12 to 17 need a BMI at or above the 95th percentile for age and sex, or at or above the 85th percentile with a severe weight-related condition. Zepbound is not approved for anyone under 18.
  • Adults 45 and older with cardiovascular disease qualify at a BMI of 27 or higher with a documented history of heart attack, stroke, or symptomatic peripheral arterial disease.
  • The patient must be actively participating in structured nutrition and physical activity programs.
  • The patient must not be pregnant or breastfeeding and must have no history of medullary thyroid cancer or multiple endocrine neoplasia type 2.

Baseline weight and BMI must be measured within 45 days of the prior authorization request. Initial approval runs six months, and renewals run 12 months with no cap on how many times they can be renewed.

What Renewal Requires

To stay on a GLP-1 for weight management, adults must have lost at least 5% of their pre-treatment weight, and adolescents must show a BMI reduction of more than 4%. A prescriber can still make the case for a patient who falls short by documenting a significant BMI reduction and maintained weight loss.7Healthy Blue NC. GLP-1 Coverage for Weight Management in Kansas

Kansas Medicaid covers GLP-1s for weight management, and Healthy Blue Kansas processes those claims with prior authorization. The state’s anti-obesity medication PA form lists Saxenda, Wegovy, and Zepbound as covered agents.9Kansas KDHE. Louisiana and South Carolina: No Weight-Loss Coverage

Healthy Blue Louisiana covers GLP-1 receptor agonists only for type 2 diabetes. The state’s clinical authorization criteria, effective November 15, 2024, do not include weight management as a covered use.2Louisiana Medicaid. South Carolina Medicaid did cover GLP-1s for weight loss starting November 1, 2024, but reversed that decision effective January 1, 2026, citing $2.3 million in state funds and $5.5 million in federal funds spent on weight-loss prescriptions in the prior fiscal year.12SC Daily Gazette. These exclusions are allowed because of a longstanding statutory exception that lets state Medicaid programs opt out of covering drugs prescribed for weight loss or weight gain.

Other FDA-Approved Uses

Separate from weight management, Healthy Blue NC covers Wegovy and Zepbound for additional FDA-approved indications, and these did not lose coverage during the 2025 gap:

  • Wegovy for reducing the risk of major cardiovascular events in adults 45 and older with established cardiovascular disease and obesity or overweight, and for treating noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced liver fibrosis.
  • Zepbound for treating moderate to severe obstructive sleep apnea in adults with obesity.

Because state Medicaid rules on GLP-1s have shifted repeatedly, the safest step before filling a prescription is to call Healthy Blue’s member services line for your state and ask whether your specific drug is covered for your specific diagnosis. A prescriber’s office can also submit the prior authorization request and confirm the current clinical criteria.
