Health Net does cover Zepbound (tirzepatide) on most of its California commercial plans, but every plan requires prior authorization, documented participation in a weight loss program, and a BMI that meets the plan’s threshold. As of January 1, 2026, that threshold jumped to 40 or higher for new prescriptions on large group employer plans and on Ambetter Individual and Family Plans. Some PPO plans exclude weight loss drugs entirely, and benefits can vary even within a plan category depending on the specific employer group.{5Health Net. If You Were Already Taking It Before January 2026
Members who were already filling Zepbound (or another covered weight loss medication) before January 1, 2026, can stay on it under the old BMI 30 (or BMI 27 with a comorbidity) rules. Two conditions attach to that protection: your prescriber has to keep an active prior authorization in place, and you have to stay enrolled in a Health Net-approved or physician-recommended lifestyle modification program.{2Health Net. The protection is fragile. If you stop filling the medication for 60 days or more, the grandfathering ends, and when you try to renew you will be judged against the new BMI 40 standard.{4Health Net. What Prior Authorization Requires
Every Health Net plan that covers Zepbound requires prior authorization. For members without type 2 diabetes seeking the drug for weight management, Health Net does not require you to fail other medications first.{5Health Net. Clinical Policy CPA.359 – Zepbound} You and your prescriber will need to show:
- You are 18 or older.
- A current height, weight, and BMI calculated within the last 30 days.
- At least six months of active participation in a qualifying weight loss program before you start Zepbound. Weight Watchers, Active&Fit, or any prescriber-recommended plan that combines a reduced-calorie diet, increased physical activity, and behavioral modification counts. - You are not taking any other GLP-1 receptor agonist or another tirzepatide product such as Mounjaro at the same time.
Initial approval runs up to 16 weeks. To renew, you have to show at least 5% weight loss at the first renewal, or maintenance of prior weight loss on later renewals.{5Health Net. Clinical Policy CPA.359 – Zepbound}
If you have type 2 diabetes, the requirements are heavier. Health Net wants documented failure of at least three consecutive months on each of Ozempic or Rybelsus (semaglutide), Trulicity (dulaglutide), generic liraglutide (Victoza), and Mounjaro before approving Zepbound, unless any of those is contraindicated or caused significant adverse effects.{5Health Net. A Separate Path if You Have Sleep Apnea
Zepbound carries an FDA-approved indication for moderate to severe obstructive sleep apnea in adults with obesity, and Health Net has a distinct coverage pathway for that use.{6U.S. Food and Drug Administration. Zepbound Prescribing Information} It matters because the BMI threshold under the OSA pathway is a flat 30 across plan types, which is lower than the weight-management threshold on most Health Net plans.
To qualify under the OSA pathway you need a confirmed diagnosis of moderate to severe OSA, with an apnea-hypopnea index of 15 or more documented in a sleep study performed within the last 12 months. You also have to show either persistent symptoms despite positive airway pressure therapy or an inability to use PAP therapy. The six-month weight loss program requirement still applies. For continued therapy on this indication, your maintenance dose has to be at least 10 mg weekly, versus 5 mg weekly on the general weight management pathway.{5Health Net. What You Will Pay
Zepbound sits on Tier 4 of Health Net’s formulary, the specialty tier.{7Health Net. Four-Tier Drug List} That means it has to be filled through an in-network specialty pharmacy, not a standard retail counter.
On large group plans, coinsurance for weight loss medications is 50%, whether your employer carries the standard benefit or the buy-up rider.{3Health Net. Weight Loss Drug Buy-Up Flyer} On a drug priced above $1,000 a month, that comes to a meaningful monthly bill. Cost sharing on other plan types depends on your benefit design; verify the specific amount with Health Net before your first fill.
Medi-Cal Members: Not Covered for Weight Loss
If your Health Net plan is a Medi-Cal managed care plan, Zepbound is no longer covered for weight loss. California’s Medi-Cal program eliminated GLP-1 coverage for adult weight loss effective January 1, 2026, and prior authorizations in effect on December 31, 2025 ended that day.{8Medi-Cal Rx. GLP-1 Changes} Medi-Cal still covers GLP-1 medications for members under 21 under federal EPSDT rules, and for adults when medically necessary for another condition such as type 2 diabetes or cardiovascular disease.{9News Medical. If Health Net Denies the Request
On commercial plans, Health Net has to issue a standard coverage determination within 72 hours of receiving your prescriber’s supporting statement, or within 24 hours if waiting could seriously harm your health.{2Health Net. Pharmacy Information for Providers} If the answer is no, you, your doctor, or an authorized representative can file an internal appeal. A strong appeal includes a detailed statement from the prescriber explaining why Zepbound is medically necessary for you specifically.
Health Net of California is regulated by the California Department of Managed Health Care. After 30 days in Health Net’s internal grievance process, or immediately in a serious threat to life or health, you can take the denial to the DMHC.{10California Department of Managed Health Care. File a Complaint} The DMHC can order an Independent Medical Review, in which an outside panel of physicians reviews the denial; if the panel disagrees with the plan, Health Net has to authorize the medication. Medi-Cal members who are denied can request a State Hearing through the California Department of Social Services within 90 days of the denial notice.
If Your Plan Won’t Cover It at All
Eli Lilly runs self-pay programs for patients whose commercial insurance excludes Zepbound. The Self-Pay Savings Card prices the 2.5 mg KwikPen dose at $299 a month and the 7.5 mg through 15 mg KwikPen doses at $449; the single-dose pen runs $499 for a one-month supply.
