What Counts as a Covered Visit

Dry needling is not a separate benefit. The per-visit three-unit cap on commercial plans applies to any mix of acupuncture codes (97810–97814) and dry needling codes (20560–20561), and on senior products the 20-visit annual cap is calculated across both.6OpenPayer. Medicare Advantage (Senior) Plans Are Different

If you are on a Harvard Pilgrim senior product, Medicare rules apply, and they are narrower. Acupuncture is covered only for chronic low back pain, defined as nonspecific pain lasting 12 weeks or longer that is not tied to surgery, pregnancy, or a systemic condition such as infection or inflammatory disease.8CMS.   • Up to 12 sessions in a 90-day initial course.
  • Up to 8 additional sessions if the patient is improving, for a total of 20 per year.
  • Providers must add a KX modifier to claims beyond the initial 12, certifying medical necessity.
  • Treatment must stop if the patient is not improving or is getting worse.

Acupuncture for other conditions, including fibromyalgia and osteoarthritis, is not covered under Medicare rules, and Harvard Pilgrim’s senior product policy incorporates those limits directly.8CMS. One Thing Acupuncture Is Not

Harvard Pilgrim offers a separate Complementary and Alternative Medicine Reimbursement benefit on certain Massachusetts group plans, worth up to $150 per family per calendar year. Acupuncture is not part of it. That reimbursement is limited to homeopathy, naturopathy, reflexology, and reiki, and the guidelines state plainly, “Other services do not qualify.”9Harvard Pilgrim Health Care. State Rules Affect What Insurers Must Offer

Harvard Pilgrim operates primarily in Massachusetts, New Hampshire, Maine, and Connecticut. Massachusetts does not currently mandate acupuncture coverage; a 2015 bill (H3972) that would have required it for pain management, PTSD, substance abuse, and nausea was reviewed by the Center for Health Information and Analysis, but the mandate is not in effect.11Center for Health Information and Analysis. Because benefits vary across plans, the only reliable confirmation comes from your own documents:

  • Pull up your Schedule of Benefits through your online account or mobile app. If acupuncture is covered, it will be listed.
