Does FSA Cover Counseling? Eligibility, Receipts, and Limits

Yes, an FSA does cover counseling when the sessions treat a diagnosed mental health condition, such as clinical depression, an anxiety disorder, PTSD, or a substance use disorder, delivered by a licensed provider. Counseling aimed at general personal growth, career direction, or relationship enhancement is not reimbursable. The IRS standard is whether the expense primarily alleviates or prevents a physical or mental illness rather than being “merely beneficial to general health.”1Internal Revenue Service. What Counseling Qualifies

IRS Publication 502 lists psychiatric care, psychologist services, and psychoanalysis as eligible medical expenses.1Internal Revenue Service. Publication 502 (2025), Medical and Dental Expenses The federal tax code treats amounts paid for medical care, including treatment of disease, as qualified expenses.2Office of the Law Revision Counsel. 26 USC 213 – Medical, Dental, Etc., Expenses In practice, that covers sessions with a psychiatrist, licensed psychologist, licensed clinical social worker, or comparable licensed professional for conditions such as:

  • Clinical depression and anxiety disorders treated through individual therapy.
  • Post-traumatic stress disorder, including treatments like Cognitive Behavioral Therapy or Dialectical Behavior Therapy prescribed for an accepted diagnosis.
  • Substance abuse and addiction treatment, inpatient or outpatient, at a therapeutic center. Meals and lodging during a residential stay qualify, and transportation to recovery support meetings such as Alcoholics Anonymous is eligible when a physician recommends attendance as part of treatment.1Internal Revenue Service. Telehealth counseling with a licensed provider is eligible as well. The federal FSA program lists telehealth and remote care as a reimbursable expense category requiring only a detailed receipt.3FSAFEDS. Eligible Health Care FSA (HC FSA) Expenses

What Does Not Qualify

Life coaching, career guidance, and self-help workshops do not treat a diagnosed condition, so the IRS treats them as personal expenses that fall outside FSA reimbursement.

Marriage and family counseling is the most common gray area. The IRS has answered it directly: therapy that treats a diagnosed mental illness qualifies, but marital counseling does not.4Internal Revenue Service. Frequently Asked Questions About Medical Expenses Related to Nutrition, Wellness and General Health If one spouse has a diagnosed condition and a licensed provider prescribes couples therapy as part of that spouse’s treatment plan, the expense could qualify. Sessions focused on relationship improvement or conflict resolution alone do not.

If your FSA debit card gets used for a non-qualifying service, your plan administrator will flag the expense and ask you to repay it or substitute another eligible expense. The improperly spent amount may be included in your taxable income for the year. The 20 percent additional tax that sometimes gets mentioned in this context applies to Health Savings Accounts, not FSAs.5Office of the Law Revision Counsel. 26 USC 223 – Health Savings Accounts

Paperwork You Will Need

Letter of Medical Necessity

A Letter of Medical Necessity (LMN) is the standard document when a service could look personal rather than medical. Your treating physician or licensed provider completes it, and the letter should include a diagnosis, a description of how counseling treats that condition, and the provider’s signature and contact information.6FSAFEDS. Letter of Medical Necessity Form It must confirm the counseling is not for general health or cosmetic purposes. Most plans treat an LMN as valid for one year, so you will need a new letter if treatment runs longer.

Itemized Receipts

Every claim should be backed by an itemized receipt showing the date of service, the provider’s name, a description of the treatment, and the amount charged. Credit card slips and account summaries alone are usually not enough.3FSAFEDS. Eligible Health Care FSA (HC FSA) Expenses Some administrators also ask for the provider’s National Provider Identifier (NPI) number. Keep all receipts and supporting documents for at least three years from the date you file your return.7Internal Revenue Service. How Long Should I Keep Records?

Paying and Getting Reimbursed

Most FSA plans give you two ways to pay. If your employer provides an FSA debit card, you can swipe it at the provider’s office. The administrator may request documentation afterward to verify the expense. If you do not have a card, or the provider does not accept it, you pay out of pocket and submit a claim.

To file a manual claim, upload your itemized receipt and any supporting documents through your plan administrator’s portal or app. Processing typically takes a few business days, after which the reimbursement is deposited into your linked bank account.

Watch the claim deadline. Expenses must be incurred during the plan year (or the grace period, if your plan offers one), and claims must be filed before the run-out period ends. Many employers set a 90-day run-out window, but this varies. Miss it and the reimbursement is forfeited.

Covering Spouses and Dependents

Your health care FSA is not limited to your own expenses. You can use it for qualifying counseling for your spouse, your tax dependents, and your children under age 27, even if those family members are not covered by your health insurance plan.8Internal Revenue Service. Publication 969, Health Savings Accounts and Other Tax-Favored Health Plans The eligibility rules are the same: the counseling must treat a diagnosed condition, and you still need an LMN where applicable plus itemized receipts.

This matters for families with children in therapy. A child’s sessions with a licensed psychologist for a diagnosed condition like ADHD or an anxiety disorder are reimbursable from your FSA the same way your own sessions would be.

FSA Type Matters If You Also Want an HSA

If you are enrolled in a high-deductible health plan and want to contribute to a Health Savings Account, be careful about which FSA you elect. A general-purpose health care FSA, the standard kind that covers all medical expenses including counseling, disqualifies you from making HSA contributions for every month you are covered by the FSA. That holds even if you spend the FSA balance early in the year.8Internal Revenue Service. Publication 969, Health Savings Accounts and Other Tax-Favored Health Plans

A limited-purpose FSA, which covers only dental and vision expenses, does not disqualify you from HSA contributions. But a limited-purpose FSA cannot reimburse mental health counseling or psychiatric care. If paying for therapy with pre-tax dollars is the priority, the choice is between a general-purpose FSA that covers counseling and an HSA paired with a limited-purpose FSA that keeps HSA eligibility intact but requires you to pay for therapy through the HSA or out of pocket.

2026 Contribution Limits, Carryover, and Grace Periods

For 2026, the maximum contribution to a health care FSA through payroll deductions is $3,400, up from $3,300 in 2025.9Internal Revenue Service. IRS Releases Tax Inflation Adjustments for Tax Year 2026, Including Amendments From the One, Big, Beautiful Bill Contributions come out of your paycheck before federal income tax and payroll taxes, which is where the savings on eligible therapy come from.

FSAs run on a use-it-or-lose-it rule: unspent funds at the end of the plan year are forfeited. Your employer may offer one of two safety valves, but not both:10Healthcare.gov. Using a Flexible Spending Account (FSA)

Your employer picks which option to offer, or neither. If you know you will be in therapy through the year, check your plan documents during open enrollment and estimate the number of sessions you expect so your election matches actual spending.
