Getting Prior Approval

Every FEP Blue plan that covers Ozempic requires prior approval. In many cases the pharmacy claim will clear automatically without a manual review if all three of these conditions are true:

  • The claim carries a type 2 diabetes diagnosis code, or you have filled at least a 30-day supply of a non-GLP-1 diabetes medication in the past 180 days.
  • The quantity is within the plan’s limit of 3 units per 84 days.
  • There is no concurrent paid claim for another GLP-1 or GIP medication.

If the claim does not clear automatically, your prescriber has to submit a prior approval request. Under the policy effective July 2025, the request has to document a type 2 diabetes diagnosis plus at least one piece of supporting clinical evidence: an HbA1c of 6.5% or higher, fasting plasma glucose at or above 126 mg/dL, or a 2-hour glucose tolerance test result at or above 200 mg/dL. How Much You Can Fill at Once

Weight Loss Use Is Not Covered

Lowering Your Cost With the Manufacturer Savings Card

Federal employees enrolled in FEP Blue can use the Novo Nordisk Ozempic Savings Card.
