Cirrhosis can qualify you for Social Security disability benefits if the liver damage is severe enough to meet the SSA’s medical criteria or to keep you from holding a job. There are two routes. The first is meeting Listing 5.05 in the SSA’s Blue Book, which sets out seven specific ways chronic liver disease can automatically qualify. The second is showing that your symptoms, treatment demands, and functional limits leave no realistic work you could sustain. Most successful cirrhosis claims take the second route.
The Two Routes to Approval
The SSA works through a five-step evaluation on every disability claim. Two of those steps do most of the work for cirrhosis applicants.
At Step 3, the SSA checks whether your condition matches Listing 5.05 for chronic liver disease. Match one of its seven pathways and you’re approved without further analysis.1Social Security Administration. Disability Evaluation Under Social Security 5.00 Digestive Disorders – Adult
If you don’t meet the listing, the analysis moves to Steps 4 and 5, where the SSA measures your residual functional capacity (RFC) against your past work and any other work in the national economy.2Social Security Administration. Code of Federal Regulations 404.1520 This is where fatigue, encephalopathy episodes, medication side effects, and time lost to appointments actually get weighed. One threshold applies before either route: if you’re earning more than $1,690 per month in 2026, the SSA treats that as substantial gainful activity and stops the claim at Step 1.3Social Security Administration. What’s New in 2026
The Seven Pathways in Listing 5.05
You only need to satisfy one. Each demands specific medical documentation, and the SSA holds the details strictly.1Social Security Administration. Disability Evaluation Under Social Security 5.00 Digestive Disorders – Adult
Variceal Hemorrhage (5.05A)
Bleeding from esophageal, gastric, or ectopic varices caused by portal hypertension qualifies if the episode caused hemodynamic instability (rapid pulse, low blood pressure, fainting, or profuse sweating) and required hospitalization with a transfusion of at least two units of blood. The bleeding must be documented by imaging. Approval under this pathway lasts one year from the transfusion, then the SSA re-evaluates.
Ascites or Hydrothorax (5.05B)
Fluid buildup in the abdomen (ascites) or around the lungs (hydrothorax) qualifies if it appears on two separate evaluations at least 60 days apart within the same 12-month period, and is not caused by something other than liver disease. Each evaluation must be documented by paracentesis or thoracentesis, or by imaging or physical exam combined with either a serum albumin of 3.0 g/dL or less, or an INR of at least 1.5.
Spontaneous Bacterial Peritonitis (5.05C)
This acute infection of the abdominal fluid qualifies when peritoneal fluid analysis shows a neutrophil count of at least 250 cells per cubic millimeter.
Hepatorenal Syndrome (5.05D)
Kidney failure triggered by liver disease qualifies with serum creatinine of at least 2 mg/dL, 24-hour urine output below 500 mL, or urine sodium below 10 mEq per liter.
Hepatopulmonary Syndrome (5.05E)
Lung involvement from liver disease qualifies when arterial blood gas testing shows oxygen at or below specific thresholds (varying by altitude), or when contrast-enhanced echocardiography or a lung perfusion scan reveals abnormal shunting.
Hepatic Encephalopathy (5.05F)
Cognitive dysfunction from liver disease qualifies when documented behavioral or mental status changes (confusion, delirium, stupor, or coma) appear on two evaluations at least 60 days apart within a 12-month period. You also need either a history of a portosystemic shunt (such as TIPS), or two evaluations 60 days apart in the same period showing one of the following: asterixis or other fluctuating neurological abnormalities, an EEG with triphasic slow waves, serum albumin of 3.0 g/dL or less, or INR of 1.5 or greater.
SSA CLD Score (5.05G)
This pathway uses a severity score the SSA developed for chronic liver disease. You need two SSA CLD scores of at least 20, obtained at least 60 days apart within a 12-month period.4Social Security Administration. SSA Chronic Liver Disease (CLD) Calculator The score is calculated from serum creatinine, total bilirubin, INR, and in some cases serum sodium. All lab values used in a single score must come from within the same 30-day window. The SSA uses the highest values for creatinine, bilirubin, and INR in that window, and the lowest value for sodium. If the initial calculation produces a score above 11, sodium is factored in for a revised score.
This scoring route catches many people with advanced cirrhosis whose complications don’t fit neatly into the other six categories. If your hepatologist tracks these labs regularly, ask whether your numbers are close to the threshold.
Qualifying Without Meeting the Listing
Most cirrhosis claims that succeed do so at Steps 4 and 5 rather than the listing. Chronic fatigue, intermittent confusion, frequent infections, medication side effects, and constant appointments can make sustained work impossible even when your lab values fall short of the listing thresholds. The RFC assessment is where those realities get counted.5Social Security Administration. 20 CFR 416.945 – Your Residual Functional Capacity
The SSA looks at how long you can sit, stand, walk, lift, concentrate, and stay on task, how often you’d need unscheduled breaks, how many days a month you’d likely miss, and whether your symptoms are predictable enough to keep a regular schedule. A detailed opinion from your treating doctor about these functional limits carries significant weight, particularly when it lines up with consistent records.
Age matters here. The SSA’s medical-vocational grid rules become more favorable at 50 and considerably more favorable at 55. An applicant over 55 who is limited to sedentary work and has no transferable job skills will generally be found disabled regardless of education. Between 50 and 54, the standards are noticeably easier than for younger applicants. If you’re in this range, make sure the RFC captures every limitation your records support.
If Alcohol Contributed to Your Cirrhosis
Alcohol is the most common cause of cirrhosis in the U.S., and it triggers a rule that catches many applicants off guard. Federal law says that if drug addiction or alcoholism is “a contributing factor material” to your disability, benefits are denied.6Office of the Law Revision Counsel. 42 USC 423
The test: would you still be disabled if you stopped drinking? If the liver damage is permanent and irreversible, the answer is yes and alcoholism is not a material factor. If your condition could improve enough for you to work with sobriety, the answer is no and benefits are denied.7Social Security Administration. Code of Federal Regulations 416.935
In practice, advanced cirrhosis with significant scarring does not reverse with sobriety, so many people with alcohol-related liver disease do qualify. The record needs to show that. Imaging or biopsy documenting the stage and permanence of the damage, along with a treating doctor’s statement that the structural damage would persist regardless of alcohol cessation, is what carries this argument. Active drinking invites closer scrutiny but is not by itself disqualifying.
The Medical Evidence That Decides Your Claim
Every cirrhosis claim lives on its medical record.8Social Security Administration. Evidentiary Requirements for Disability Evaluation The evidence falls into a few categories.
Lab work is the backbone. The SSA looks at liver function panels, bilirubin, serum albumin, INR and other coagulation studies, serum creatinine, serum sodium, and platelet counts. For the SSA CLD score pathway, creatinine, bilirubin, INR, and sodium must all come from the same 30-day window, and you need two qualifying sets at least 60 days apart.
Imaging and procedures document the physical state of the liver, the presence of varices, and fluid accumulation. Ultrasound, CT, MRI, and endoscopy all contribute. Biopsy results showing the degree of fibrosis carry particular weight because they directly demonstrate irreversible damage.
Hospital records for variceal bleeding, encephalopathy episodes, infections, or paracentesis are powerful evidence, and the SSA pays close attention to how often those events occur over time.
A treating physician’s statement ties the labs to the reality of your day. What you cannot do, how often symptoms flare, how treatments affect your energy and thinking. This becomes especially important when your case rests on the RFC rather than the listing.
SSDI or SSI: Which Program Applies
The medical standard for cirrhosis is the same in both programs, but the non-medical eligibility rules are different.9Social Security Administration. Overview of Our Disability Programs
SSDI is tied to your work history. You earn credits through Social Security payroll taxes, one credit for every $1,890 in covered earnings in 2026, up to four credits a year.10Social Security Administration. Social Security Credits and Benefit Eligibility If you’re 31 or older, you generally need at least 20 credits earned in the 10 years before your disability began. Your benefit amount is based on your lifetime earnings.
SSI has no work history requirement. It’s needs-based. In 2026, countable resources cannot exceed $2,000 for an individual or $3,000 for a couple.11Social Security Administration. 2026 Cost-of-Living Adjustment (COLA) Fact Sheet Your primary home and one vehicle don’t count; bank accounts, investments, and additional property do. The maximum federal SSI payment in 2026 is $994 per month for an individual and $1,491 for a couple, with some states adding a supplement.12Social Security Administration. SSI Federal Payment Amounts for 2026 You can apply to both programs at once, and some people qualify for both.
Applying
You can apply online at ssa.gov, by calling 1-800-772-1213, or in person at your local Social Security office.13Social Security Administration. How Do I Apply for Social Security Disability Benefits Before you start, pull together your medical providers’ contact information, a current medication list, your work history for the past 15 years, and recent lab and hospital records.
After you file, the state’s Disability Determination Services office reviews the medical evidence and may contact your doctors or schedule a consultative exam if the record has gaps. Roughly 63% of initial applications are denied. That number reflects how the process is designed to filter at the front end more than the quality of individual claims, so an initial denial is not the end.
Appealing a Denial
You have 60 days from receiving the denial to appeal, and the SSA assumes you received the notice five days after the letter date, so in practice you have 65 days from the date on the letter.14Social Security Administration. Request Reconsideration There are four levels.15Social Security Administration. Appeal a Decision We Made
- Reconsideration. A different reviewer examines the full file, including any new evidence. Most are still denied.
- Hearing before an administrative law judge. You or your representative appear before a judge, present evidence, and answer questions. Approval rates at this stage are significantly higher than at the initial level, and many cirrhosis claims are won here.
- Appeals Council review. The Council can grant, deny, or send the case back to a judge.
- Federal district court. The final step.
You can bring in an attorney or accredited representative at any stage, though most people do so at the hearing level. Representatives work on contingency, paid only if you win, with the fee capped by federal rule and withheld directly from any back pay.16Social Security Administration. Fee Agreements For claims that turn on the RFC assessment or the alcohol rule, someone who knows how to frame the medical evidence and question a vocational expert at the hearing can change the outcome.
Liver Cancer and Faster Processing
Cirrhosis alone is not on the SSA’s Compassionate Allowances list, but hepatocellular carcinoma is.17Social Security Administration. Complete List of Conditions – Compassionate Allowances If your cirrhosis has progressed to primary liver cancer, name the cancer diagnosis clearly on the application to trigger expedited review.
