CHAMPVA does cover GLP-1 medications, but only when prescribed for specific FDA-approved diagnoses. It will not cover any GLP-1 drug prescribed for weight loss, no matter which drug is on the prescription. Whether your claim goes through depends on the exact medication and the diagnosis your provider records.
Which GLP-1 Drugs CHAMPVA Covers, and For What
Each covered GLP-1 is tied to a particular condition. If the diagnosis on file doesn’t match, the pharmacy won’t fill it.1OptumRx. CHAMPVA Landing Page
- Type 2 diabetes: Mounjaro (tirzepatide), Ozempic (semaglutide injection), Rybelsus (oral semaglutide), Trulicity (dulaglutide), and Victoza (liraglutide).
- Moderate to severe obstructive sleep apnea: Zepbound (tirzepatide). The sleep apnea must be verified by a sleep study conducted within the past 12 months.
- Metabolic-associated steatohepatitis (MASH) or prevention of major adverse cardiovascular events (MACE): Wegovy (semaglutide). Either qualifying diagnosis is enough on its own.2VA.gov. CHAMPVA Care
Why Weight-Loss Prescriptions Are Refused
CHAMPVA states plainly that it “doesn’t cover prescription GLP-1 medications for weight loss.”3VA.gov. Meds by Mail for CHAMPVA and Other Family Member Programs The OptumRx page for CHAMPVA beneficiaries lists Saxenda, Wegovy, and Zepbound as not covered when prescribed as weight-loss medications.1OptumRx. The distinction matters because Wegovy and Zepbound are covered drugs. Wegovy goes through when prescribed for MASH or cardiovascular risk reduction, and Zepbound goes through when prescribed for sleep apnea. The same drug written for obesity or weight management will be rejected. The diagnosis on the prescription is what determines whether the claim is paid.
What Changed on January 1, 2025
Starting January 1, 2025, CHAMPVA tightened coverage for two of the most commonly prescribed GLP-1 drugs. Mounjaro and Ozempic are now covered only for beneficiaries with a confirmed type 2 diabetes diagnosis.4VA.gov. The same update formalized the current diagnosis-specific framework for Zepbound and Wegovy.
Prior Authorization for Wegovy and Zepbound
Two of the covered GLP-1 drugs require prior authorization before CHAMPVA will approve the claim: Wegovy and Zepbound. Providers submit authorization requests to OptumRx at 844-403-1029.2VA.gov. CHAMPVA Care
For Zepbound, the provider must document moderate to severe obstructive sleep apnea confirmed by a sleep study within the past 12 months. For Wegovy, the qualifying diagnosis of MASH or MACE prevention must be established. Additional details on the preauthorization process are in the CHAMPVA Guidebook (updated March 2026).
The GLP-1 drugs covered for type 2 diabetes (Mounjaro, Ozempic, Rybelsus, Trulicity, and Victoza) do not appear to require prior authorization based on available program materials, but the VA recommends calling 800-733-8387 to confirm coverage for any specific medication.4VA.gov. How to Actually Get the Prescription Filled
CHAMPVA beneficiaries have two channels, and the cost difference between them is significant.
Meds by Mail
Meds by Mail is the VA’s home-delivery pharmacy for CHAMPVA, and prescriptions filled through it have no out-of-pocket cost.5VA.gov. New prescriptions take up to 21 days to process and deliver. Refills ordered by phone or online take up to 10 days; refills by mail take up to 15 days. One limitation to know: if you have other health insurance with prescription drug coverage, including Medicare Part D, you can’t use Meds by Mail.5VA.gov. CHAMPVA Care
OptumRx Retail Pharmacies
For urgent prescriptions, or when Meds by Mail doesn’t work, you can use retail pharmacies in the OptumRx network. Most pharmacies participate.6OptumRx. If the pharmacy is outside the network, you pay the full cost upfront and are later reimbursed for 75%.5VA.gov. CHAMPVA Care
Because GLP-1 medications can cost $350 to $500 per month at retail without insurance, a 25% cost share is still substantial. CHAMPVA caps total household out-of-pocket costs at $3,000 per calendar year, and covers 100% of eligible expenses after that.5VA.gov. CHAMPVA Care
