CDPHP does cover Zepbound (tirzepatide) for weight management on its commercial HMO and PPO, health exchange, and Managed Medicaid plans, but every one of those plans requires prior authorization and step therapy, and you’ll need to document at least three consecutive months in a comprehensive weight management program before approval.1PrescriberPoint. Clinical Criteria You Have to Meet
For a new Zepbound prescription, CDPHP will only authorize the drug if you fall into one of two BMI categories:
- BMI of 40 or greater (Class 3 obesity), with no additional conditions required.
- BMI of 35 to 39.9 (Class 2 obesity), with documented weight-related comorbidities.

On top of the BMI threshold, you must have been actively enrolled in a comprehensive weight management program for at least the three months immediately before the request. The program has to include diet modification or nutrition education, an exercise component, behavior modification, and regular individual coaching or group sessions. If you’re already on Zepbound and want continued coverage, your provider has to submit your starting weight, current weight, and documentation that you’re still enrolled in a qualifying weight management program.
How the Prior Authorization Gets Submitted
Your prescribing provider submits a Prior Authorization/Medical Exception Request Form to the CDPHP Pharmacy Department by mail or fax. The form asks for diagnosis, drug information, dosing, and a history of prior therapies or adverse reactions, along with the clinical documentation above.2CDPHP. Medicare Advantage Members: The GLP-1 Bridge Program
CDPHP’s Medicare Advantage formulary does not list Zepbound or tirzepatide for weight management, because federal law has long prohibited Medicare Part D from covering drugs prescribed specifically for weight loss.3CDPHP. Starting July 1, 2026 and running through December 31, 2027, a federal Medicare GLP-1 Bridge Program creates a separate pathway. It covers the Zepbound KwikPen (single-dose vials and pens are excluded) at a flat $50 monthly copay, which does not count toward your Part D deductible or out-of-pocket limit.5Medicare.gov. The Bridge Program runs through a central Medicare system, not through CDPHP. You’re not eligible if you already receive GLP-1 medications through your standard Part D plan, or if you have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease, since standard Part D may already cover a GLP-1 for those conditions.5Medicare.gov. Medicaid Members and NYRx
CDPHP’s Managed Medicaid formulary lists Zepbound with prior authorization and step therapy,1PrescriberPoint. Zepbound Prior Authorization – Capital District Physicians Health Plan but since April 2023, all CDPHP Medicaid members get prescription drug coverage through NYRx, New York’s Medicaid Pharmacy Program, rather than through CDPHP.6CDPHP. If CDPHP Denies Your Request

Medicare Advantage members follow a different route. First, request a “coverage determination” for a formulary exception by submitting a Coverage Determination Request Form to the CDPHP pharmacy department. If that’s denied, the next step is a “redetermination,” a formal review by the appeals department. You can also file a complaint directly with Medicare.8CDPHP. What You’ll Pay
Zepbound’s list price runs roughly $1,086 for a 28-day supply of four prefilled pens. What you actually pay depends on your plan’s cost-sharing. Commercial CDPHP members with Zepbound covered may qualify for Eli Lilly’s Zepbound Savings Card, which can drop the copay to as little as $25 per fill, up to $1,950 in annual savings.9Drugs.com.
