Does Blue Cross Blue Shield Cover Hotel Stays? Limits and Claims

Blue Cross Blue Shield plans can cover hotel stays, but only in narrow situations. Whether Blue Cross Blue Shield covers a hotel depends on why you’re traveling, how far the facility is from your home, and whether your particular plan or employer added a travel benefit in the first place. The most common trigger is an organ or bone marrow transplant at a designated center. Some plans go further and cover lodging for cancer treatment, bariatric surgery, cardiac care, gender-affirming care, pregnancy termination, and other specialty services. A standard commercial plan with no travel rider will not pay for a hotel just because you drove a long way to see a doctor.

When a Hotel Stay Is Actually Covered

Transplants are the one scenario where lodging coverage shows up consistently. The Federal Employee Program covers hotel costs for members receiving care at a Blue Distinction Center for Transplants, up to $5,000 per transplant for the member and companions combined, as long as the member lives at least 50 miles from the facility and gets prior approval.¹ Blue Cross NC runs a similar transplant-only benefit with a $10,000 cap.²

Several plans have expanded well beyond transplants. Blue Cross Blue Shield of Michigan’s Travel Concierge program covers lodging for behavioral health, cancer treatment, cardiac care, maternity and reproductive health, musculoskeletal procedures, gender affirmation care, bariatric surgery, rare conditions, and pregnancy termination.³ BlueCross BlueShield of Tennessee offers transplant travel across all plan types and, for self-funded employer groups that opt in, extends lodging to Blue Distinction Specialty Care procedures including bariatric surgery, cancer care, cardiac care, fertility care, joint replacement, spinal surgery, and substance use treatment.⁴

A different category of coverage targets care that members can’t legally get at home. Blue Shield of California reimburses hotel stays when a member has to travel out of state for services restricted in their home state, currently pregnancy termination and gender-affirming care.⁵ Anthem Blue Cross in California has offered small-group employers a comparable rider covering out-of-state travel when necessary care is unavailable locally, capped at $3,000 per year.⁶

How Far From Home You Have To Be

Every plan with a lodging benefit sets a minimum distance between your home and the treating facility. The thresholds don’t match across plans or even across services within the same plan.

Blue Cross NC uses 100 miles for solid organ transplants and 30 miles for bone marrow and stem cell transplants.²
BlueCross BlueShield of Tennessee applies 30 miles for transplants and 50 miles for Blue Distinction Specialty Care procedures.⁴
The FEP transplant benefit and BCBS of Mississippi’s transplant lodging policy both use 50 miles.¹⁷
BCBS of Massachusetts sets the general travel benefit at 100 miles, and BCBS of Mississippi uses 100 miles for its network access benefit when specialty care isn’t locally available.⁸⁹

In some plans the threshold is set by the employer rather than the insurer. The Michigan Travel Concierge program tells members to call customer service to find out what minimum distance their specific plan uses.³

How Much the Plan Will Pay

Nightly caps are modest. Overall caps are higher but still well short of what an extended stay in a major city can cost.

BCBS of Massachusetts: $50 per person per night, or $100 with a companion. Employer-specific limits can apply on top of that.⁸
Michigan Travel Concierge: $50 per approved traveler per night, keyed to IRS guidelines.³
Excellus BCBS: $50 per night solo, $100 with a companion, capped at $4,000 per year for total travel and lodging.¹⁰
BCBS of Michigan Medicare Plus Blue Group PPO: $150 per day combined for member and companion. Lifetime caps of $10,000 for solid organ transplants and $5,000 for clinical trials or bone marrow transplants.¹¹
BCBS of Mississippi transplant benefit: $200 per day combined for member and companion, $10,000 aggregate per transplant episode.⁷
BCBS of Mississippi network access benefit: federal GSA per diem rate or actual cost, whichever is lower, with a $10,000 annual cap covering all travel expenses.⁹
Blue Shield of California: up to $250 per day for a double-occupancy room, $3,000 annual maximum.¹²
FEP: $5,000 total per transplant covering both lodging and transportation.¹

Normal cost-sharing like copays and deductibles usually doesn’t apply to travel reimbursements. If you have an HSA-qualified high-deductible plan, that’s the exception: you generally have to meet the deductible first so the travel benefit doesn’t disqualify your HSA.³¹²

What the Hotel Bill Can and Cannot Include

Across BCBS plans the lodging benefit covers the room charge, and sometimes room tax. That’s it. Meals, alcohol, entertainment, laundry, phone charges, personal items, and room service are consistently excluded.⁷⁸ Childcare, pet boarding, lost wages, security deposits, and tips are also out. The FEP plan specifically excludes food under IRS rules.¹

Short-term rentals are a separate wrinkle. BCBS of Massachusetts and the Michigan Travel Concierge limit reimbursement to hotels and motels, so Airbnb-style stays won’t qualify.⁸³ BCBS of Mississippi accepts apartments and short-term home rentals for transplant travel, but won’t pay for staying with friends or family.⁷

A few plans do reimburse meals separately. Blue Shield of California pays up to $100 per day for meals.¹² BCBS of New Mexico’s Medicaid plan reimburses meals at $22.92 per day when a member is away from home for eight or more hours.¹³

Getting Approval and Filing the Claim

Start by calling the member services number on the back of your ID card. The call confirms two things that matter before you book anything: whether the travel benefit is actually part of your plan, and what rules apply. BCBS of Massachusetts, for instance, directs members to call 1-888-420-4501 to verify whether their employer opted in.⁸

Prior authorization is standard. BCBS of Mississippi wants at least two days’ notice for network access travel.⁹ Michigan Travel Concierge pairs members with a concierge who helps pick a provider and arrange travel and lodging rather than leaving the booking to the member.³

After the trip, submit the plan-specific reimbursement form with itemized receipts. Filing deadlines differ sharply: 60 days for BCBS of New Mexico Medicaid members,¹⁴ 90 days at BCBS of Mississippi,⁹ and up to 18 months at Blue Cross NC.² Michigan Travel Concierge pays within 30 days of approval.³

Medicaid and Medicare BCBS Plans

If your BCBS coverage comes through Medicaid or Medicare, the rules look different. Blue Cross Complete of Michigan, a Medicaid plan, covers lodging and meals as part of mileage reimbursement when members drive to appointments, but trips involving lodging must be approved through MTM Health with at least two days’ notice.¹⁵ BCBS of New Mexico’s Turquoise Care Medicaid plan pays for lodging when a trip exceeds four hours one way and an overnight stay is medically necessary, but the member cannot book the hotel. All arrangements go through ModivCare, and a self-booked hotel will not be reimbursed.¹⁴

On the Medicare side, BCBS of Michigan’s Medicare Plus Blue Group PPO includes travel and lodging as an enhanced benefit for organ transplants and clinical trials. It applies only to group PPO members whose employer bought the enhanced coverage, not to individual Medicare PPO members.¹¹

Why Two BCBS Members Get Different Answers

Blue Cross Blue Shield is a federation of independent companies. Each state’s plan sets its own policies, and within any one of them the travel benefit is often an optional rider that an employer can elect or decline. A self-funded employer in Tennessee might add Blue Distinction Specialty Care lodging coverage for its workers while a fully insured individual plan in the same state has nothing comparable.⁴ BCBS of Massachusetts spells out that its travel benefit exists only when the employer opts in.⁸

Before booking any trip you expect your plan to pay for, call member services, confirm the travel benefit is in your plan, pin down the distance threshold and dollar caps, and get prior authorization in writing. Without that, a hotel receipt is just a hotel receipt.
