How Many Visits Your Plan Will Pay For

There is no system-wide BCBS visit limit. Caps run from about 10 to 30 visits a year, and some plans have no fixed cap but require review after a certain point. A few documented examples:

An in-network chiropractor almost always costs significantly less. In-network providers have agreed to accept the plan’s negotiated allowable amount, so your share is capped at the copay or coinsurance. Prior Authorization

Many commercial PPO plans do not require authorization for a routine chiropractic visit. Referrals

PPO plans generally do not require a referral from a primary care doctor. HMO and managed-care plans are more likely to. Medicare Advantage Plans

Original Medicare (Part B) covers only manual spinal manipulation to correct a subluxation. It does not pay for X-rays ordered by the chiropractor, extraspinal adjustments, therapeutic modalities, or maintenance care. BCBS Medicare Advantage plans start from that baseline and can add benefits on top. Why Coverage Varies So Much

Blue Cross Blue Shield is not one company. It is an association of 33 independent, locally operated companies, each setting its own policies within its territory. Three other factors shape what any specific plan covers.

State Mandates

Many states require insurers to cover chiropractic care. If a Chiropractic Claim Is Denied
