Does Anthem Blue Cross Cover Tirzepatide? Mounjaro, Zepbound, Appeals

Anthem Blue Cross does cover tirzepatide, but only in specific circumstances. Mounjaro, the version approved for type 2 diabetes, is covered under most Anthem plans as a preferred brand with prior authorization. Zepbound, the version approved for weight management, is excluded from most Anthem plans unless the prescription is tied to obstructive sleep apnea or a state mandate requires coverage. Mounjaro for Type 2 Diabetes

Anthem plans generally cover Mounjaro when it is prescribed for type 2 diabetes, and on at least some formularies it appears as a Tier 2 preferred brand.2Formulary Navigator. To get prior authorization approved, your prescriber usually needs to document a type 2 diabetes diagnosis supported by at least one of the following lab findings: a hemoglobin A1c of 6.5% or higher, a fasting plasma glucose of 126 mg/dl or higher, a two-hour plasma glucose of 200 mg/dl or higher on an oral glucose tolerance test, or symptoms of hyperglycemia with a random plasma glucose of 200 mg/dl or higher.4Anthem Provider News. Zepbound for Weight Loss

Zepbound shares the same active ingredient as Mounjaro, but it is labeled for chronic weight management in adults with a BMI of 30 or greater, or 27 or greater with at least one weight-related condition such as high blood pressure, high cholesterol, or type 2 diabetes.5U.S. Food and Drug Administration. Most Anthem plans do not cover Zepbound for weight loss. In California, Medi-Cal Rx removed Zepbound from the Contract Drugs List entirely as of January 1, 2026, and claims deny automatically regardless of indication.3Anthem Providers. GLP-1 Coverage Policy Commercial and employer-sponsored Anthem plans tend to exclude weight loss medications as well, though employer plans vary widely because the employer, not Anthem, often decides whether to include anti-obesity drug coverage at all.

The Sleep Apnea Pathway

In December 2024, the FDA approved Zepbound for the treatment of moderate to severe obstructive sleep apnea in adults with obesity.6U.S. Food and Drug Administration. Anthem’s Medi-Cal Rx policy in California, for example, states that prior authorization requests for Zepbound will be considered when the drug is used for OSA, even though Zepbound is otherwise off the drug list.3Anthem Providers. The clinical criteria plans typically use for this indication include a confirmed diagnosis of moderate to severe OSA by sleep study (an apnea-hypopnea index of 15 or more events per hour), a BMI of 30 or greater, documented compliance with first-line OSA treatment such as CPAP, and evidence of at least six months in a weight-loss program without achieving a 5% or greater reduction in body weight.7Advanced Health. Why Two People With the Same Anthem Card Get Different Answers

Many large employers self-insure their health benefits and simply hire Anthem to process claims. In those arrangements the employer, not Anthem, decides which drug categories are covered. A single employee on a GLP-1 drug can cost a plan $12,000 to $16,000 per year, so employers worried about cost often drop weight-loss drug coverage. Even inside the same company, a higher-tier plan may cover a drug that a lower-tier plan excludes.

Fully insured Anthem plans, the kind sold on the individual marketplace or to small businesses, follow formularies Anthem sets centrally and state insurance rules. Those formularies still differ by state, by product line (HMO, PPO, EPO), and by year. The only reliable way to find out what your plan covers is to check the formulary through the member portal at anthem.com, read your Summary of Benefits and Coverage for an explicit exclusion of anti-obesity medications, or have your prescriber run a real-time benefits check through the pharmacy system.

Medicare Advantage and Part D

If your Anthem plan is a Medicare Advantage plan with Part D, the rules are different. Part D covers Mounjaro when it is prescribed for type 2 diabetes, with prior authorization and quantity limits.2Formulary Navigator. CMS has launched the Medicare GLP-1 Bridge Program, running July 1, 2026 through December 31, 2026, which provides temporary access to Zepbound, Wegovy, and Foundayo for weight loss outside the regular Part D benefit. Beneficiaries pay a $50 monthly copayment, and those costs do not count toward Part D deductibles or out-of-pocket limits.9Centers for Medicare & Medicaid Services. State Mandates That Can Change the Default

A few states have moved to require coverage. North Dakota became the first to mandate GLP-1 and GIP drug coverage by amending its Essential Health Benefit benchmark plan, effective January 1, 2025. The mandate names semaglutide and tirzepatide and applies to non-grandfathered individual and small group market plans in the state.11North Dakota Insurance Department. Most other state efforts have stalled. During the first half of 2025, at least 14 states introduced bills or took regulatory action on GLP-1 coverage.12Pharmacy Times. Appealing an Anthem Denial

If Anthem denies your prior authorization, you have the right to appeal. The denial letter will give the reason: medical necessity not established, diagnosis not meeting criteria, formulary exclusion, or step therapy not met are the common ones. You generally have 180 days from the denial date to file an internal appeal. Medicare Advantage members use a process called a plan redetermination. Requests can come from you, your appointed representative, or your prescriber, through forms on Anthem’s website, electronically through CoverMyMeds, by fax to 888-458-1406, or by mail to Anthem’s Appeals and Grievances Department in Mason, Ohio.14Anthem. Appeals go better with the right documentation attached: a letter of medical necessity from your prescriber, current BMI and comorbidity records with ICD-10 codes, proof that preferred alternatives failed or caused side effects, and documentation of lifestyle efforts such as diet, exercise, or a weight management program. Your physician can also request a peer-to-peer review, a direct conversation with Anthem’s medical director. If the internal appeal is denied, you can request an independent external review. An outside third party evaluates the case, the review is free, and if they rule in your favor Anthem must cover the drug. The deadline is generally about four months from the final internal denial, though it varies by state. Under CMS rules effective in 2026, insurers must give transparent denial reasons and meet stricter timelines: 72 hours for urgent requests and seven days for standard ones.

If Your Plan Won’t Pay

If you have commercial Anthem coverage that includes tirzepatide, Eli Lilly’s savings cards can bring the copay to as little as $25 per month for both Mounjaro and Zepbound.15Eli Lilly. Zepbound Savings These programs expire December 31, 2026, and are not available to anyone on Medicare, Medicaid, TRICARE, VA, or other government-funded insurance.
