Aetna Medicare plans do not cover weight loss drugs when they are prescribed for weight loss. The restriction applies to every anti-obesity medication, including GLP-1 drugs like Wegovy and Zepbound and older oral drugs like Contrave and Qsymia. It is not an Aetna decision. Federal law bars all Medicare Part D plans from paying for drugs used for weight loss, so no Part D plan or Medicare Advantage plan with drug coverage will fill a weight loss prescription. Starting July 1, 2026, a federal pilot called the Medicare GLP-1 Bridge Program offers a workaround: eligible patients can get Wegovy, Zepbound, or Foundayo for a flat $50 monthly copay, outside their Aetna plan.
Why the Ban Exists
The exclusion comes from the Medicare Prescription Drug, Improvement and Modernization Act of 2003, which created Part D and incorporated a Medicaid-based list of drugs that Part D plans may exclude. One category on that list is “agents when used for anorexia, weight loss, or weight gain.”1Office of the Law Revision Counsel. 42 USC 1395w-102 CMS reads this as a prohibition, and in the final rule for Contract Year 2026 the agency said it did “not intend to finalize proposals to reinterpret the statutory exclusion on weight loss drugs.”2Applied Policy. A bill called the Treat and Reduce Obesity Act of 2025, introduced in both chambers (S. 1973 and H.R. 4231), would amend the statute to allow Part D coverage of FDA-approved obesity drugs.3Congress.gov. When Aetna Does Cover These Drugs
The same medications your plan refuses for weight loss can be covered when prescribed for a different FDA-approved condition. What matters is the diagnosis on the prescription.
Aetna’s Part D and Medicare Advantage plans cover Ozempic and Mounjaro for type 2 diabetes. Aetna’s parent company, CVS Health, has confirmed coverage of Wegovy for cardiovascular risk reduction in patients with established heart disease who meet body weight criteria, consistent with CMS guidance.5JR Report. The Medicare GLP-1 Bridge Program
CMS launches the Medicare GLP-1 Bridge Program on July 1, 2026. It is a federal demonstration that sits outside Part D entirely. Aetna is not involved in approving, dispensing, or paying for Bridge prescriptions; a central processor run by Humana handles all of it.8CMS. Which Drugs Are Covered
- Wegovy (semaglutide), injectable pens and oral tablets.
- Zepbound (tirzepatide), KwikPen only. Single-dose pens and vials are not covered.
- Foundayo (orforglipron), a once-daily oral GLP-1 pill from Eli Lilly approved by the FDA in April 2026, which can be taken any time of day without food or water restrictions.9Eli Lilly. FDA Approves Lillys Foundayo
Who Qualifies
You must be at least 18 and enrolled in a Medicare plan with prescription drug coverage, which includes most Aetna Medicare Advantage plans and standalone Aetna Part D plans. The clinical criteria look at your BMI and health conditions at the time you first started therapy:10CMS. Medicare GLP-1 Bridge – Information for Providers
- BMI of 35 or higher: no additional conditions required.
- BMI of 30 or higher: with heart failure with preserved ejection fraction, uncontrolled hypertension despite two medications, or chronic kidney disease stage 3a or above.
- BMI of 27 or higher: with pre-diabetes, a history of heart attack or stroke, or symptomatic peripheral artery disease.
If you have type 2 diabetes, moderate-to-severe sleep apnea, or MASH, you are not eligible for the Bridge, because those conditions already open coverage through your Aetna Part D benefit.11CMS. GLP-1 Bridge Prescriber Information Sheet
How to Get a Prescription Filled
You do not enroll separately. Your doctor writes the prescription and the pharmacy submits it to the Bridge’s central processor using a dedicated billing code. That triggers a prior authorization request, which your prescriber completes to attest to your BMI and qualifying conditions. Approvals or denials come back within 72 hours.11CMS. What You Pay and What Does Not Apply
The copay is a flat $50 per monthly prescription. Only 28- or 30-day fills are covered. That $50 does not count toward your Part D deductible or the $2,100 annual out-of-pocket cap, and the Low-Income Subsidy (Extra Help) does not apply to Bridge prescriptions.12Medicare Rights Center. What Comes After the Bridge
CMS designed a follow-on program called BALANCE (Better Approaches to Lifestyle and Nutrition for Comprehensive Health) that would fold weight loss drug coverage into regular Part D plans on a voluntary basis. The Medicare side of BALANCE required 80 percent of Part D sponsors, weighted by enrollment, to sign on for a January 2027 launch. Some reporting has described BALANCE as “delayed indefinitely” for Medicare, though its Medicaid component launched in May 2026.17Health Affairs. Buying Out of Pocket Through TrumpRx
If you are not eligible for the Bridge or want access before July 2026, you can pay cash. The federal TrumpRx portal, launched in early 2026, lists prices negotiated under “most-favored nation” agreements with Novo Nordisk and Eli Lilly.18The White House. Weight Management Benefits in Your Aetna Plan
Some Aetna Medicare Advantage plans include supplemental weight management benefits that do not involve prescription drugs. Through the Aetna Personal Health Solutions portal, eligible members can use programs like Weight Watchers and Habitnu (a CDC-developed diabetes prevention and weight loss program) at no additional cost.20Aetna Personal Health Solutions. Medicare Part B Counseling and Surgery
Original Medicare covers obesity screening and behavioral counseling for beneficiaries with a BMI of 30 or higher, with up to 22 sessions in a year at no cost.21Humana. HSA and Tax Deduction
If you still have a Health Savings Account, you can withdraw funds tax-free to pay for a prescribed weight loss medication even when your Part D plan does not cover it. Prescribed obesity medications may also qualify as a tax-deductible medical expense if you itemize and your total qualified medical expenses exceed 7.5 percent of your adjusted gross income.7AARP.
