A child with non-mosaic Down syndrome automatically qualifies for SSI on the medical side, but SSI is a needs-based program, so the family still has to clear the income and resource rules before payments start. The disability determination itself is essentially settled by a karyotype report; the rest of the application is about finances.
What “Automatic” Actually Means
The Social Security Administration lists non-mosaic Down syndrome, covering both Trisomy 21 and translocation forms, in Section 110.06 of its Blue Book of childhood impairments. The proof the SSA needs is a karyotype analysis, the chromosome test that confirms the diagnosis. Either a lab report signed by a physician is enough, or an unsigned lab report accompanied by a separate physician’s statement confirming the diagnosis.1Social Security Administration. 110.00 Congenital Disorders That Affect Multiple Body Systems Most children are tested shortly after birth, so parents usually already have the document when they apply.
So when people say Down syndrome is an “automatic” qualifier for SSI, that is accurate for the medical listing, and only for the medical listing. The financial test is separate and is where most denials happen.
Mosaic Down Syndrome Is Handled Differently
Mosaic Down syndrome, which affects about 2% of children with Down syndrome, is not on the automatic list. If no specific listing fits, the SSA runs a “functional equivalence” analysis across six domains: acquiring and using information, attending and completing tasks, interacting with others, moving and manipulating objects, self-care, and health and physical well-being. Children with mosaic Down syndrome often qualify, but the path is evidence-driven rather than automatic.
The Part That Isn’t Automatic: Income and Resources
SSI is need-based, so the SSA also weighs household income and assets.3Social Security Administration. Not everything counts. The SSA subtracts allowances for the parents and for other children in the household, and it excludes certain kinds of income entirely. Only what remains is deemed to the child. Larger households pick up more deductions, so moderate-income families can and do qualify.
The resource limits are stricter and have not moved in decades. A child’s countable resources cannot exceed $2,000. When parental resources are calculated, the first $2,000 is excluded for a single-parent household and $3,000 for a two-parent household; anything above those thresholds counts against the child’s $2,000 cap.5Social Security Administration. What the Payment Looks Like
The maximum federal SSI payment for an eligible individual in 2026 is $994 per month, reflecting a 2.8% cost-of-living adjustment.7Social Security Administration. SSI Federal Payment Amounts Many states add a supplement, so the total varies by location. If deeming leaves the child with countable income, the actual payment will be less than the federal maximum.
Presumptive Payments While the Claim Is Reviewed
Down syndrome is one of the conditions where the SSA can authorize presumptive disability payments during the first visit to a field office, before the formal determination is finished.8Social Security Administration. Presumptive payments run for up to six months while the full decision is pending. If the claim is later denied for any reason, those payments do not have to be repaid.9Social Security Administration. The online disability application is aimed at adults, so a child’s claim generally means a phone or in-person interview.10Social Security Administration. For non-mosaic Down syndrome, the karyotype does most of the work on the disability side. The other documents mostly support the financial review.
Initial decisions generally take six to eight months, though non-mosaic Down syndrome claims with a clean karyotype often move faster because no functional analysis is required.13Social Security Administration. What Changes at Age 18
During the year after your child turns 18, the SSA redetermines eligibility using the adult disability rules rather than the childhood standard.14Social Security Administration. Non-mosaic Down syndrome has its own listing in the adult Blue Book as well. The financial side often gets easier. Parental deeming stops at 18, so only the young adult’s own income and resources count.4Social Security Administration.
