Do Retired Military Get Free Healthcare: TRICARE Costs by Age

Retired military members do not get completely free healthcare, but they receive heavily subsidized coverage through TRICARE that costs far less than comparable civilian plans. Retirees under 65 choose between two TRICARE plans with annual enrollment fees ranging from roughly $187 to $1,191 depending on the plan and when they entered service, plus copayments for office visits and prescriptions.1TRICARE Newsroom. Learn Your 2026 TRICARE Health Plan Costs At 65, retirees transition to TRICARE for Life, which wraps around Medicare and eliminates most remaining out-of-pocket costs, but it requires paying the standard Medicare Part B premium of $202.90 per month in 2026.2Centers for Medicare and Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles

Standard retiree TRICARE eligibility requires 20 years of active service or a medical retirement. Members separated with a disability rating below 30 percent do not qualify for standard retiree coverage. Guard and Reserve retirees who qualify with 20 years generally cannot access TRICARE Prime or Select until age 60, and that gap has its own cost picture covered below.

What Retirees Under 65 Pay

Your cost depends on two things: the plan you choose and when you entered the military. If your initial enlistment or commissioning was before January 1, 2018, you fall into Group A. On or after that date puts you in Group B, which generally pays more.3TRICARE. Beneficiary Groups

TRICARE Prime

Prime works like an HMO. A primary care manager coordinates your care and refers you to specialists; seeing a specialist without a referral triggers steep point-of-service charges. Prime is only available in established network areas, so rural retirees may not have the option.

Prime has no annual deductible. Primary care copays are $26 in 2026 for both Group A and Group B. Annual enrollment fees for 2026 are:1TRICARE Newsroom. Learn Your 2026 TRICARE Health Plan Costs

  • Group A: $381.96 individual, $765 family
  • Group B: $462.96 individual, $927 family

If you go outside your primary care manager for care, point-of-service charges apply: a $300 individual deductible ($600 family) in 2026, then a 50 percent cost-share of the TRICARE-allowable charge. Those amounts do not count toward your catastrophic cap, so they can pile up quickly.4TRICARE. TRICARE 2026 Costs and Fees Sheet

TRICARE Select

Select works more like a PPO. You can visit any TRICARE-authorized provider without a referral. The trade-off is a deductible and higher copays. A network primary care visit costs $38 for Group A and $33 for Group B in 2026. Non-network visits carry a 25 percent cost-share after the deductible.4TRICARE. TRICARE 2026 Costs and Fees Sheet

Select deductibles for 2026:1TRICARE Newsroom. Learn Your 2026 TRICARE Health Plan Costs

  • Group A: $150 individual, $300 family (network and non-network combined)
  • Group B: $198 individual and $397 family for network care; $397 individual and $794 family for non-network care

The pattern to notice: Group A retirees usually pay less overall with Select if they use little care. Group B retirees generally pay more for Select than for Prime.

The Catastrophic Cap

TRICARE limits total out-of-pocket spending per family each calendar year. Once you hit the cap, which includes enrollment fees, deductibles, and copayments, TRICARE covers all additional authorized costs for the rest of the year. The specific amount depends on your group and plan and is listed on the annual TRICARE Costs and Fees Sheet.4TRICARE. TRICARE 2026 Costs and Fees Sheet

What Prescriptions Cost

Every TRICARE plan includes pharmacy coverage. Filling prescriptions at a military treatment facility pharmacy is free. TRICARE Pharmacy Home Delivery covers up to a 90-day supply at these 2026 copays:5TRICARE Newsroom. Preview Your 2026 TRICARE Pharmacy Costs

  • Generic formulary: $14
  • Brand-name formulary: $44
  • Non-formulary: $85

Retail network pharmacies charge more than home delivery, and non-network pharmacies charge more still and may require you to pay upfront and file for reimbursement. If you take a maintenance medication, moving it to home delivery or a military pharmacy is one of the easiest ways to cut annual costs.

What Retirees Pay at 65 and After

At 65 your coverage changes. TRICARE for Life (TFL) replaces Prime and Select and acts as a supplement wrapped around Medicare. Federal law requires enrollment in both Medicare Part A and Medicare Part B to keep any TRICARE coverage at all.6Office of the Law Revision Counsel. 10 USC 1086 – Contracts for Health Benefits for Certain Members, Former Members, and Their Dependents Skip Part B and you lose TRICARE entirely.

TFL itself has no enrollment fee and no separate premium.7TRICARE Newsroom. For covered services, you typically owe nothing.

Higher-Income Retirees Pay More for Part B

Higher-income retirees pay an Income-Related Monthly Adjustment Amount (IRMAA) on top of the standard Part B premium. The 2026 tiers for individual filers:2Centers for Medicare and Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles

  • $109,001–$137,000: $284.10 per month total
  • $137,001–$171,000: $405.80 per month total
  • $171,001–$205,000: $527.50 per month total
  • $205,001–$499,999: $649.20 per month total
  • $500,000 or more: $689.90 per month total

Joint filer thresholds are roughly double. IRMAA is based on modified adjusted gross income from two years earlier, so a one-time event like a property sale can bump you into a higher tier for a year. If retirement itself triggered the income spike, you can appeal.

Guard and Reserve Retirees Before Age 60

Guard and Reserve members who accumulate 20 qualifying years cannot enroll in TRICARE Prime or Select until they reach age 60.8Military Compensation and Financial Readiness. Reserve Retirement That age can drop as low as 50 for members recalled to active duty after January 28, 2008, with a three-month reduction for each cumulative 90 days of qualifying active service.9Defense Finance and Accounting Service. Gray Area Retirees – Army Reserve and National Guard The stretch between retiring from service and reaching eligible age is called the “gray area.”

During the gray area, coverage is available through TRICARE Retired Reserve (TRR), a premium-based plan the government does not subsidize. You pay the full cost. In 2026, monthly premiums are:1TRICARE Newsroom. Learn Your 2026 TRICARE Health Plan Costs

  • Member only: $645.90 per month
  • Member and family: $1,548.30 per month

That is closer to civilian-market pricing than to standard retiree TRICARE, though it may still beat comparable individual-market plans for retirees with families or pre-existing conditions. Once you reach the eligible age and start drawing retired pay, you have 90 days from your birthday to enroll in Prime or Select.10TRICARE. Retiring from the National Guard or Reserve

What TRICARE Doesn’t Cover

Retiree TRICARE does not include dental or vision benefits. For those, you buy separate coverage through the Federal Employees Dental and Vision Insurance Program (FEDVIP), and you pay the full premium yourself. There is no government subsidy for retirees.11U.S. Office of Personnel Management. Dental and Vision Monthly costs vary by carrier, plan tier, and location; you can compare options through the BENEFEDS portal.12BENEFEDS. Dental and Vision Carriers and Plan Options Enrollment is limited to the annual federal benefits open season or a qualifying life event.

Lowering What You Actually Pay

Three moves cut retiree costs beyond what the plan structure alone provides.

If you have employer coverage, TRICARE by law pays second when you have other health insurance.13TRICARE. Using Other Health Insurance The employer plan processes claims first, and TRICARE covers qualifying remaining costs. Coordination often reduces out-of-pocket to near zero. One caution: if your employer plan denies a claim because you skipped its rules, such as a required pre-authorization, TRICARE can deny it too.

If you are enrolled in VA healthcare for service-connected conditions, you can use it alongside TRICARE. All VA facilities participate as TRICARE network providers.14VA/DoD Health Affairs. VA and TRICARE Information Splitting care by source, VA for rated conditions and TRICARE for the rest, often minimizes total spending.

And where a military treatment facility is nearby, using its pharmacy for maintenance medications is free. Over a year, that alone can offset most of a Prime enrollment fee.
