Do I Need to Report Form 5498 on My Tax Return?

You do not need to report Form 5498 on your tax return. Your IRA custodian files it directly with the IRS and sends you a copy for your records, but the form is informational only. You don’t attach it to your return, and you don’t wait for it to file. In fact, you can’t wait: custodians have until the end of May to issue Form 5498, so it almost always arrives after the April filing deadline. You report your IRA contributions, rollovers, and conversions on your return using your own records, then use Form 5498 as a cross-check when it shows up.

Why You Don’t Wait for the Form

Form 5498 covers contributions made during the calendar year and through the following April filing deadline. That’s the whole reason custodians get until May 31 to send it. For the 2025 tax year, because May 31, 2026 falls on a Saturday, the deadline shifts to June 1, 2026.1Internal Revenue Service. 2025 Instructions for Forms 1099-R and 5498

By then, your return is already filed. So the sequence is: file first, verify later. The IRS eventually matches what you reported against the Form 5498 your custodian submits, which is why your return and the form need to agree.

How to Report IRA Activity Without the Form

Use your own records to figure out what to put on your return: confirmation emails, contribution receipts, year-end statements, and bank or brokerage records showing the dates and amounts you funded your account.

Deductible Traditional IRA Contributions

If you qualify to deduct a traditional IRA contribution, claim it on Schedule 1 of Form 1040, on the line labeled “IRA deduction.”2Internal Revenue Service. Schedule 1 (Form 1040) Whether you can take the full deduction, a partial one, or none depends on your income and whether you or your spouse are covered by a retirement plan at work.

Nondeductible Contributions and Roth Contributions: File Form 8606

If you make a nondeductible contribution to a traditional IRA, you need to file Form 8606, Nondeductible IRAs, with your return. Form 8606 tracks your basis, meaning the after-tax dollars you’ve already paid tax on. Without that record, the IRS has no way to know which portion of a future withdrawal is already taxed and which isn’t.3Internal Revenue Service. 2025 Instructions for Form 8606 – Nondeductible IRAs

Skipping Form 8606 is costly. If you never establish your basis, the IRS may treat your entire distribution as taxable income in retirement. On top of that, there’s a $50 penalty for each year you fail to file the form, unless you can show reasonable cause.4Office of the Law Revision Counsel. 26 U.S. Code 6693 – Failure to Provide Reports on Certain Tax-Favored Accounts or Annuities The $50 looks small, but years of missed forms become a much bigger problem when you start taking distributions and can’t prove what you already paid tax on.

Roth Conversions

If you converted traditional, SEP, or SIMPLE IRA money to a Roth during the year, the converted amount is included in your taxable income for that year. You report the conversion on Form 8606.3Internal Revenue Service. 2025 Instructions for Form 8606 – Nondeductible IRAs A conversion is not the same as a Roth contribution, and the two show up in different boxes on Form 5498 (Box 3 for conversions, Box 10 for direct Roth contributions). Keep them straight on your return.

Rollovers

Money properly rolled over from another retirement account into an IRA stays tax-deferred. On Form 5498, rollovers appear in Box 2. If a rollover gets mistakenly coded as a regular contribution, it can look like you blew past the contribution limit, so categorization matters.

What to Check When Form 5498 Arrives

When the form comes in May or June, compare each box against the numbers you used on your return. Priority items:

  • Box 1 (traditional IRA contributions) matches what you deducted or reported.
  • Box 10 (Roth IRA contributions) matches what you contributed directly to a Roth.
  • Box 2 (rollovers) and Box 3 (Roth conversions) are coded correctly, not lumped in as contributions.
  • Box 4 (recharacterizations) reflects any contribution you redesignated from one IRA type to the other.
  • Box 5 (fair market value as of December 31) looks right against your year-end statement.

A rollover mistakenly reported as a contribution is the classic cause of an IRS notice, because the agency’s records will show you contributing far more than the annual limit.

If There’s an Error on the Form

Contact your custodian right away and ask them to file a corrected Form 5498. The IRS has specifically flagged custodian errors on Form 5498 as a source of trouble for taxpayers.5Internal Revenue Service. Form 5498 – Errors by IRA Trustees, Issuers and Custodians May Cause Tax Trouble If your return already used the correct figures, you don’t need to amend it. You just need the custodian to fix their filing so the IRS’s records match yours.

If the Form Shows a Problem on Your Return

Sometimes the form is right and the return is wrong. Maybe you deducted a contribution you weren’t eligible to deduct, or you contributed more than the annual limit. In that case, you may need to amend. An accuracy-related penalty of 20% can apply to any underpayment from claiming deductions or credits you didn’t qualify for, though the IRS can waive it for reasonable cause.6Internal Revenue Service. Excess contributions have their own problem. Contributing more than your allowed limit triggers a 6% excise tax on the excess, and that tax hits every year the excess stays in the account. To avoid it, withdraw the excess contributions and any earnings on them before your tax filing deadline, including extensions.7Internal Revenue Service. Retirement Topics – IRA Contribution Limits

Required Minimum Distributions Flagged on Form 5498

Form 5498 also tells you whether you owe a required minimum distribution for the coming year. Box 11 is a checkbox: if it’s marked, you need to take an RMD. Box 12a shows the deadline, and Box 12b shows the calculated amount. If Box 11 is checked but Box 12b is blank, your custodian must send you the amount separately or offer to calculate it by January 31.8Internal Revenue Service. RMDs generally start the year you turn 73. Your first RMD can be delayed until April 1 of the following year, but that means two RMDs land in the second year. After that, each year’s RMD is due by December 31.9Internal Revenue Service. Retirement Plan and IRA Required Minimum Distributions FAQs

Missing an RMD, or taking too little, is expensive. The penalty is 25% of the amount you should have taken but didn’t, dropping to 10% if you correct the shortfall within two years.10Internal Revenue Service. Retirement Topics – Required Minimum Distributions (RMDs) Even though the custodian reports your RMD status on Form 5498, the responsibility for taking the correct distribution is yours.

A Note on Recharacterizations

If you contributed to one type of IRA and then changed your mind, a recharacterization lets you treat the contribution as if it had gone to the other type from the start. The recharacterized amount, including allocable earnings, appears in Box 4. You must complete the recharacterization by your tax filing deadline, including extensions; if you filed on time but missed that window, you have until six months after the original due date (October 15 for most people) to make the transfer and file an amended return.11Internal Revenue Service. Publication 590-A (2025), Contributions to Individual Retirement Arrangements (IRAs)

Recharacterization applies only to contributions. You cannot recharacterize a Roth conversion back into a traditional IRA; the Tax Cuts and Jobs Act eliminated that option starting in 2018.

One Boundary for SEP and SIMPLE IRAs

If you have a SEP or SIMPLE IRA, employer contributions show up in Box 8 (SEP) or Box 9 (SIMPLE) rather than in Box 1. The timing is also different: traditional and Roth contributions in Boxes 1 and 10 include amounts contributed through the April filing deadline, but SEP and SIMPLE amounts in Boxes 8 and 9 reflect only what was deposited during the calendar year itself.8Internal Revenue Service. Form 5498 IRA Contribution Information 2025 If you’re expecting a January or February employer contribution for the prior year, it won’t appear on that year’s Form 5498.

The bottom line stays the same across all IRA types: Form 5498 is not a return document. File on time with your own records, then check the form when it arrives and act on any mismatch.
