There is no single disability certificate form. What people call disability certificate forms is really a family of documents, each tied to a specific program: Social Security benefits, ABLE savings accounts, FMLA leave, ADA workplace accommodations, state parking placards, state disability insurance, VA compensation, or India’s national UDID system. The form you need, who has to sign it, and what it must say all depend on what you are trying to access.

Social Security Disability (SSDI and SSI)
Applying for SSDI or SSI does not involve one physician-signed certificate. The Social Security Administration uses a package of forms, and a state agency called Disability Determination Services makes the actual finding based on the medical evidence it gathers.
Three forms carry the process:
- Form SSA-16, the Application for Disability Insurance Benefits, collects identification, work and earnings history, the date the condition became disabling, other benefit filings such as workers’ compensation, and family details. - Form SSA-3368-BK, the Disability Report — Adult, runs nearly fifteen pages across twelve sections covering medical conditions, all current medications, every treating provider, a five-year work history with detailed physical-demand questions, and education. - Form SSA-827 is that consent. It authorizes providers, schools, and other record holders to release information to SSA and DDS, is valid for twelve months from signature, covers substance abuse treatment records, and complies with HIPAA and FERPA.3Social Security Administration. DDS reviews the medical evidence and, if it is insufficient, may arrange a government-funded consultative examination. The agency then decides whether the applicant meets the legal definition of disability: inability to perform any work for which the person is suited, with a condition expected to last at least twelve months or result in death.4Social Security Administration. A central reference in the review is SSA’s Listing of Impairments, known as the Blue Book. It sets out conditions severe enough to establish disability automatically, organized into fourteen adult body-system categories with a separate set of childhood listings. Meeting or equaling a listing is generally enough; not meeting one does not end the claim, because the adjudicator moves to further evaluation steps.5Social Security Administration. ABLE Account Disability Certification
ABLE accounts are tax-advantaged savings accounts authorized under Section 529A of the Internal Revenue Code. The ABLE Age Adjustment Act, effective January 1, 2026, raised the age-of-onset threshold from 26 to 46, so anyone whose qualifying disability began before their 46th birthday can now open an account.6ABLE National Resource Center. If you already receive SSI, SSDI, or Disabled Adult Child benefits for a disability that began before age 46, you generally do not need a separate certificate — your SSA benefit statement is the proof.7ABLE National Resource Center. The ABLE National Resource Center publishes a sample form. It must be signed by a licensed doctor of medicine, osteopathy, dental surgery, podiatric medicine, optometry, or chiropractic, who certifies either a severe medically determinable impairment causing marked and severe functional limitations expected to last at least twelve continuous months (or result in death), or blindness under the SSA definition. The physician must also confirm the condition began before the individual’s 46th birthday, give a primary diagnosis with the ICD code, and certify that the impairment meets, medically equals, or functionally equals a condition in the SSA Listing of Impairments or Compassionate Allowance Conditions.8ABLE National Resource Center. 2026 ABLE Disability Certification
Most ABLE plans rely on self-certification at enrollment and do not require the form to be uploaded. Keep the signed statement in your records anyway, because the plan or the IRS may ask for it.7ABLE National Resource Center. ABLE Account Disability Certificate
FMLA Medical Certification (WH-380-E and WH-380-F)
The Family and Medical Leave Act gives eligible employees up to twelve weeks of unpaid, job-protected leave per year for a serious health condition. Employers can require medical certification, and the Department of Labor provides two optional forms: WH-380-E for the employee’s own condition and WH-380-F for a family member’s.9U.S. Department of Labor. WH-380-E asks the health care provider for three things. Part A covers the condition: approximate start date, probable duration, and the category of serious health condition (inpatient care, incapacity requiring treatment, pregnancy, chronic condition, permanent or long-term condition, or conditions requiring multiple treatments). Part B addresses the leave itself, including planned treatment dates, whether a reduced schedule is medically necessary, and the estimated frequency and duration of intermittent episodes. Employers may design their own certification forms but cannot ask for information beyond what FMLA regulations allow, and they must accept certifications in any format, including on a provider’s letterhead. Employees get at least fifteen calendar days to return a completed form, and an insufficient certification can lead to denial. ADA Accommodation Documentation
The Americans with Disabilities Act does not use a standardized disability certificate. When an employee requests a reasonable accommodation and the disability or the need is not obvious, the employer may ask for medical documentation. EEOC guidance sets limits: the employer can request information about the nature, severity, and duration of the impairment, the activities it limits, and why the requested accommodation is needed, but it cannot demand complete medical records or details of unrelated conditions.11U.S. Equal Employment Opportunity Commission. Documentation can come from physicians, psychiatrists, psychologists, nurses, physical and occupational therapists, or vocational rehabilitation specialists. If the employer requires an examination by its own provider because initial documentation is insufficient, the employer pays for it, and the exam must be limited to the disability and accommodation at issue.11U.S. Equal Employment Opportunity Commission. The Job Accommodation Network, a service of the Department of Labor’s Office of Disability Employment Policy, publishes a template titled “Medical Inquiry in Response to an Accommodation Request.” It has four sections: disability determination, accommodation necessity, effective accommodation options, and additional comments. It includes a mandatory GINA notice telling the provider not to include genetic information. State Disability Parking Certificates
Disability parking placards are issued at the state level, and each state has its own application form, qualifying conditions, and renewal rules. Minnesota is a workable example.
In Minnesota, the applicant fills out page one and a qualified health professional — physician, physician assistant, advanced practice registered nurse, chiropractor, or physical therapist — completes and signs page two, certifying at least one qualifying condition. Qualifying conditions include Class III or IV cardiac conditions under the American Heart Association classification, use of portable oxygen, severe respiratory disease, loss of a limb without a functional prosthetic, legal blindness, reliance on a wheelchair or walking aid, and inability to walk 200 feet without stopping, life-threatening aggravation, or significant fall risk. Permanent certificates run six years and renew without a new health professional signature, subject to random audits. Temporary and short-term certificates carry a five-dollar fee and require re-certification. Fraudulent certification by the applicant or the health professional is a misdemeanor with a $500 fine.13Minnesota Department of Public Safety. Disability Parking Certificate Application

California State Disability Insurance
California’s State Disability Insurance program, run by the Employment Development Department, uses Form DE 2501, the Application for Disability Insurance Benefits. The claim has two parts: Part A, completed by the claimant, and Part B, the Physician/Practitioner’s Certificate. EDD will not process a claim until both are received.14California EDD. Physicians submit Part B through myEDD’s SDI Online system or on paper. The form must show diagnoses, ICD codes, and an estimated recovery date; “unknown” and “indefinite” are not accepted. Initial claims must be filed within 49 days of the date the disability begins. A wide range of licensed professionals can certify: physicians, chiropractors, podiatrists, optometrists, dentists, psychologists, nurse practitioners, physician assistants, and midwives (for pregnancy and postpartum conditions).15California EDD. VA Disability Rating Decision Letters
Veterans do not fill out a separate certificate to prove disability status. The Department of Veterans Affairs issues a decision letter after ruling on a claim, and that letter is the official certification. It typically lists the claimed conditions and their disability ratings as percentages, effective dates, a combined evaluation, monthly compensation, payment start dates, and dependency information.16Department of Veterans Affairs. Veterans can download decision letters through VA.gov by going to the disability section and selecting “Check your claim or appeal status.” Paper copies are still mailed. The date on the letter matters: veterans have one year from it to file a timely appeal.17Department of Veterans Affairs. Download VA Letters
Disability Certification in India (UDID)
India’s certification system is governed by the Rights of Persons with Disabilities Act of 2016 and the RPwD Rules of 2017, which recognize 21 categories of disability. These range from locomotor disabilities (including cerebral palsy, muscular dystrophy, and acid attack survivors) to visual and hearing impairments, intellectual disabilities, autism spectrum disorder, specific learning disabilities, mental illness, chronic neurological conditions such as Parkinson’s disease, and blood disorders including sickle cell disease.18National Center for Biotechnology Information. Certification now runs largely through the Unique Disability ID portal at swavlambancard.gov.in, administered by the Department of Empowerment of Persons with Disabilities. Applicants submit online (offline applications are accepted and digitized), are assigned to a hospital for assessment by specialist doctors, and appear before a Medical Board that determines the type and percentage of disability. The Board’s assessment produces both a disability certificate and a UDID card. The RPwD Rules prescribe specific forms. Form V covers conditions such as amputation, paralysis, dwarfism, and blindness. Form VI covers multiple disabilities and requires documentation of each affected body system, a specific diagnosis, the percentage of impairment for each of the 21 categories, and whether the condition is progressive. Form VII covers individual disabilities other than dwarfism and blindness. Form VIII is issued when an application is rejected.20Securities and Exchange Board of India. Eligibility for government benefits requires “benchmark disability,” meaning 40% or more. Below that threshold, an applicant receives a certificate documenting the condition but does not qualify for reservations or concessions. Benefits at or above the threshold include reservations in higher education (5%) and government employment (4%), income tax rebates, railway fare concessions, free travel on state transport, subsidized prosthetic devices, and various scholarship and insurance schemes.18National Center for Biotechnology Information. Workers’ Compensation and Private Disability Insurance
Two more contexts are worth flagging so you don’t assume the forms above cover them. Workers’ compensation uses physician-completed forms that document whether a disability is total or partial, temporary or permanent, and related to a workplace injury or illness; the specific form varies by state and insurer. Private short-term and long-term disability policies use their own insurer-specific certifications, and the standard turns on the policy’s definition of disability, which can range from inability to perform your own occupation to inability to perform any occupation.21American Academy of Family Physicians. What Makes These Forms Actually Work
Across every one of these programs, the form translates a medical reality into an administrative decision. The physician has to give specific, objective clinical findings, not vague or conclusory statements. Claims get denied regularly because certifications lack sufficient detail, and that pattern holds whether the form is for Social Security, a parking placard, or a workplace accommodation.21American Academy of Family Physicians.
