To apply for federal disability benefits you file three core forms with the Social Security Administration: Form SSA-16 (Application for Disability Insurance Benefits), Form SSA-3368 (Adult Disability Report), and Form SSA-827 (Authorization to Disclose Information). Veterans applying for service-connected compensation use a separate form, VA Form 21-526EZ, and workers in a handful of states file additional forms for short-term state disability programs. The disability benefit application forms below cover what each one collects, who signs it, and where it fits in the process.
Form SSA-16: Application for Disability Insurance Benefits
Form SSA-16 is the primary application for Social Security Disability Insurance (SSDI). It was updated in September 2025, and the SSA estimates completion takes about 20 minutes.1Social Security Administration. Form SSA-16
The form collects:
- Personal information: name, Social Security number, date and place of birth, citizenship status.
- Employment and earnings history for recent years.
- The date the medical condition became severe enough to prevent work.
- Details about current and past marriages, and information on dependent children.
- Any other public disability benefits received, such as workers’ compensation or Veterans benefits.

You sign under penalty of perjury. If you sign by mark rather than a written signature, two witnesses are required. After filing, you have an ongoing obligation to report changes to the SSA: returning to work, changes in marital status, confinement in a correctional facility for more than 30 days, absences from the United States of 30 or more consecutive days, and any improvement in your medical condition.1Social Security Administration. Form SSA-3368: Adult Disability Report
Form SSA-16 handles the administrative side of the application. The Adult Disability Report is where you describe your medical conditions and work history in your own words, and it is what state Disability Determination Services use to develop the medical and vocational evidence needed to decide your claim.3Social Security Administration. The form asks about every physical and mental condition limiting your ability to work. For each treating source, it wants names and contact information for doctors, hospitals, clinics, and therapists, along with dates and purposes of visits, medications and their prescribing physicians, and the results of medical tests.
It also requires a detailed work history covering jobs held in the 15 years before your disability began: job duties, physical demands like lifting and standing, hours worked, and rates of pay.4Social Security Administration. Form SSA-827: Authorization to Disclose Information
No disability application is considered complete without a signed medical release. Form SSA-827 authorizes the SSA and state disability agencies to collect medical, educational, and employment records from your providers.6Social Security Administration. SSA-827 Information Page
The authorization is broad. It covers mental health treatment, substance abuse treatment, HIV/AIDS records, educational records under FERPA, and employer or workers’ compensation files.7Social Security Administration. Form SSA-827 The form is valid for 12 months from the date you sign it, and you can sign electronically through the online application or on paper in blue or black ink. The SSA processes over 14 million information requests annually using this form.6Social Security Administration. SSA-827 Information Page
You can try to restrict what the form covers by crossing out certain text, but the SSA warns this can delay processing. Any restrictions have to be made on the paper version with a handwritten signature; the electronic version does not allow modifications.8Social Security Administration. Which Program Your Forms Are For: SSDI, SSI, or Both
The forms above serve two federal programs that share a definition of disability but differ in who qualifies.
SSDI is funded through payroll taxes and requires enough work credits. For 2026, a worker earns one credit for every $1,890 in wages, up to four credits per year, and generally needs 40 credits with 20 earned in the last decade.9NCOA. Both programs use the same disability standard: a severe medical condition preventing substantial work that is expected to last at least 12 months or result in death. Some applicants file for both simultaneously (concurrent benefits). This typically happens when an SSDI payment is low enough to also meet SSI’s income limits; in 2026, an SSDI benefit above roughly $1,014 per month will generally rule out SSI on top.11AARP. How to Submit the Forms
The SSA accepts disability applications three ways: online, by phone, and in person at a local Social Security office.12Social Security Administration. Apply for Disability Benefits There is no fee to apply. A family member, friend, or representative may help you set up an appointment or complete the forms.
Online
The online application is open to applicants who are 18 or older, are not currently receiving Social Security benefits on their own record, have a medical condition expected to last at least 12 months or result in death, and have not been denied disability benefits within the last 60 days.12Social Security Administration. The online path is narrower for SSI. To file for SSI concurrently online, you must be between 18 and 65, never married, never previously an SSI applicant or recipient, and a U.S. citizen living in the 50 states, D.C., or the Northern Mariana Islands.13Social Security Administration. By Phone or in Person
You can call the SSA at 1-800-772-1213 (TTY 1-800-325-0778) Monday through Friday, 7 a.m. to 7 p.m., to schedule an appointment. Free interpreter services are available in over 200 languages.2Social Security Administration. Application for Social Security Disability Benefits
One detail worth knowing: if you call to schedule an appointment and then keep it, the SSA may use the date of your original phone call as the official filing date, which can affect when your benefits begin. If you miss the appointment, you have 60 days from a follow-up letter to file and still preserve that earlier date. Documents to Have Ready Before You File
The forms move faster when the information is already in front of you. The SSA publishes an Adult Disability Starter Kit with a checklist, a medical and job worksheet, and a fact sheet.15Social Security Administration. Adult Disability Starter Kit The checklist asks you to gather:
- Date and place of birth, Social Security number, spouse and former spouse details (including SSNs and marriage or divorce dates), and the names and contact information for two people who know about your medical conditions.
- Any medical records you already have, plus the names, addresses, phone numbers, and treatment dates for all doctors, hospitals, clinics, and therapists.
- A list of current medications with prescribing providers, and a list of medical tests with dates and the provider who ordered each.
- Jobs held in the years before the disability, with dates, hours, and earnings; highest level of education; any special education or vocational training.
- W-2 forms or self-employment tax returns; bank routing and account numbers for direct deposit; and any workers’ compensation details including settlement agreements, dates of injury, and claim numbers.15Social Security Administration. Adult Disability Starter Kit
The SSA requires objective medical evidence from an acceptable medical source to establish a medically determinable impairment.16Social Security Administration. Evidentiary Requirements If the existing evidence is not enough, the SSA will arrange and pay for a consultative examination, and in some cases covers travel costs.14Social Security Administration. VA Form 21-526EZ for Veterans
Veterans seeking disability compensation for conditions related to military service use a separate system run by the Department of Veterans Affairs. The primary form is VA Form 21-526EZ, Application for Disability Compensation and Related Compensation Benefits, which can be filed online through the VA’s portal or submitted on paper.17U.S. Department of Veterans Affairs. VA claims require three core elements: evidence of a current disability, documentation of an in-service event or injury, and a medical nexus connecting the two.18U.S. Department of Veterans Affairs. Evidence Needed Supporting documents typically include DD-214 separation papers, service treatment records, doctors’ reports and test results, and lay statements from fellow service members (buddy statements) submitted on VA Form 21-10210.
Certain conditions call for additional forms. VA Form 21-0781 is used for PTSD and mental health claims, and VA Form 21-8940 is used for individual unemployability claims.18U.S. Department of Veterans Affairs. State Short-Term Disability Forms
A handful of states run their own mandatory short-term disability programs for temporary, non-work-related illnesses and injuries. These are separate from SSDI and SSI and use their own forms and deadlines. Most states have no equivalent program.
California
California’s State Disability Insurance (SDI), administered by the Employment Development Department (EDD), is filed primarily through SDI Online in the myEDD portal. Paper filers use the Claim for Disability Insurance Benefits form, DE 2501.19California Employment Development Department. File no earlier than 9 days and no later than 49 days after the disability begins. A physician must certify the disability within 49 days of the start date by completing Part B of the DE 2501. There is a seven-day unpaid waiting period, with the first payable day being the eighth. Eligibility determinations generally take up to 14 days after both parts arrive, and citizenship and immigration status do not affect SDI eligibility.19California Employment Development Department. New York
Most New York employers must carry disability benefits insurance. Claimants file Form DB-450, which has three parts: Part A for the claimant, Part B for the healthcare provider (including an estimated return-to-work date), and Part C for the employer.21NYSIF. Filing a Claim Submit the completed form to your most recent employer or the New York State Insurance Fund within 30 days of becoming disabled. Benefits equal 50% of average gross wages over the preceding eight weeks, capped at $170 per week, for up to 26 weeks in a 52-week period, with an eight-day waiting period.
New Jersey, Rhode Island, and Hawaii
New Jersey’s Temporary Disability Insurance pays up to 85% of average weekly wages, capped at $1,119 per week for 2026, for up to 26 weeks, with claims filed within 30 days of becoming disabled. Rhode Island calculates benefits at 4.62% of wages in the highest quarter of the base period, up to $1,103 per week, for up to 30 weeks, and requires filing within 90 days. Hawaii requires employers to provide temporary disability coverage (through private insurance, self-insurance, or collective bargaining plans) paying 58% of average weekly wages up to $871 per week for up to 26 weeks.22Triage Health. What Trips Up Disability Applications
Incomplete or inaccurate medical evidence is the most frequent problem. The SSA requires documentation detailed enough to establish the nature and severity of an impairment, how long it has lasted or is expected to last, and what work-related activities you can still perform.16Social Security Administration. Evidentiary Requirements Claims are delayed or denied when medical records contain unresolved conflicts or inconsistencies, when the treating medical source cannot be recontacted to clarify findings, when a provider lacks the equipment to perform required tests, or when the medical source is not qualified under SSA regulations.
A doctor’s opinion that you are disabled is considered as part of the evidence but is not enough on its own to qualify you for benefits.15Social Security Administration. Applicants who describe their symptoms in detail, including how conditions affect daily activities, the frequency and intensity of symptoms, what triggers them, and what medications or treatments they use, give the SSA the fullest picture to evaluate.16Social Security Administration.
