The DD Form 363AF is the Air Force Certificate of Retirement, a ceremonial document presented to retiring airmen at the end of their careers. It recognizes your service formally, but it is not the paperwork you use to claim veterans’ benefits or prove your service to an employer. That job belongs to the DD Form 214. Think of the 363AF as the certificate you frame; the 214 is the one you file.
Who Receives It

Members who retire for physical disability under Chapter 61 of Title 10 follow a different process and may have fewer than 20 years of service. Whether they receive a DD Form 363AF depends on the circumstances of the medical retirement; your servicing Military Personnel Flight and DAFI 36-3203 can confirm what applies.
One boundary worth flagging: the 363AF is for Air Force retirements only. Members of the United States Space Force receive DD Form 363SPF instead.3Air Force’s Personnel Center. Retirement
How the Certificate Is Prepared and Presented
You do not fill it out yourself. Once your retirement application is approved, your local Military Personnel Flight pulls the required data from your master personnel record: full legal name, final permanent grade, and total creditable service.3Air Force’s Personnel Center. Most retiring members receive the certificate at a formal ceremony at their duty station. Commanders are expected to recognize every retiree with a ceremony. Why This Is Not Your DD Form 214
People trip up here often, so it’s worth stating plainly. The 363AF is ceremonial. The DD Form 214, Report of Separation, is the working document that proves your service dates, discharge characterization, and eligibility for VA benefits, veterans’ preference in federal hiring, and state veteran programs. Employers, the VA, and government agencies will ask for the 214. Secure a copy before you leave active duty; replacement is possible later through the Air Force Personnel Center but slower.3Air Force’s Personnel Center. Retirement
Requesting a Replacement
If your original certificate is lost or destroyed, you can request a replacement. The preferred method is Standard Form 180, Request Pertaining to Military Records, submitted to the National Personnel Records Center. You can also start the request online through the National Archives eVetRecs system at vetrecs.archives.gov, which uses ID.me for identity verification.6National Archives. Request Military Service Records Whichever route you use, be ready to provide:
- Your complete name as used during service
- Service number and Social Security number
- Branch of service
- Dates of service
- Date and place of birth, especially if your service number is unknown
A next of kin requesting the certificate for a deceased retiree must include proof of death, such as a death certificate or published obituary. All requests, online or paper, must be signed and dated by the veteran or the authorized next of kin.6National Archives. Correcting an Error on the Certificate
If the certificate shows the wrong rank, wrong dates, or other factual errors, the fix runs through the Air Force Board for Correction of Military Records. Applications go through the Air Force Review Boards Agency online portal at afrba-portal.cce.af.mil. Include supporting evidence such as promotion orders, performance reports, or official correspondence that documents the specific error. The stronger the paper trail, the faster the board can act. Processing takes time; plan for a wait of several months at minimum, and expect the board to request more information along the way.
