DA PAM 738-751: Forms, Status Codes, and PMCS Records

DA PAM 738-751 is the Department of the Army pamphlet that tells aviation units exactly how to document maintenance, usage, warranty actions, and equipment deficiencies under the Army Maintenance Management System – Aviation, known as TAMMS-A.1Google Books. Ground vehicles and weapons systems are not covered here. Those follow the same TAMMS logic under a separate pamphlet, DA PAM 750-8. If your equipment does not fly or support something that flies, you are working out of the wrong pamphlet.

What the Pamphlet Governs

AR 750-1 requires every Army organization to maintain its equipment and report its condition.2Aviation Assets. It defines the forms, entries, and reporting actions that produce an auditable trail from the operator’s daily checks through depot-level overhaul.

AR 750-1 organizes Army maintenance into two levels: field (operator, crew, unit, and direct support work) and sustainment (formerly general support and depot).2Aviation Assets. A fault discovered by a crew chief and a component rebuilt at a depot end up in the same equipment record, so the history is continuous no matter where the work happens.

The pamphlet also folds specialized programs into the same paperwork stream. Oil analysis findings, warranty claims, and modification work orders all get captured through TAMMS-A forms rather than running on parallel tracks.

How the Pamphlet Works Inside GCSS-Army

The pamphlet describes paper-based forms as the procedural baseline, but the day-to-day execution now happens inside the Global Combat Support System – Army. GCSS-Army implements the TAMMS-A procedures electronically. It generates the same forms and captures the same data points the pamphlet prescribes.

That shift did not retire the pamphlet. When a soldier opens a work order in GCSS-Army, the data fields trace back to the form instructions in DA PAM 738-751. The pamphlet remains the authoritative answer to what has to be recorded and why. Knowing it is what separates a soldier who clicks through screens from one who can spot a bad entry before it corrupts unit readiness data.

The Forms You Will Actually Use

A handful of forms carry most of the documentation load. Learn these and the rest of the system starts to make sense.

DA Form 2404 and DA Form 5988-E

The DA Form 2404, Equipment Inspection and Maintenance Worksheet, is the paper form used to list deficiencies and the actions taken to correct them. Its electronic version, the DA Form 5988-E, does the same job inside GCSS-Army and is what most soldiers see today. Both forms record the equipment’s nomenclature, model, serial number, operating data such as hours, and the technical manual used for the inspection. Each deficiency gets a fault code that drives the equipment’s official readiness status.

DA Form 5990-E

Once a fault is recorded on the 5988-E, the DA Form 5990-E converts it into a maintenance request that tasks maintainers to fix it. Priority designators on the request set how quickly the work is scheduled. High-priority requests require authentication by the unit commander or a designated representative. Lower-priority requests cover deferred maintenance, cosmetic repairs, and modification work orders where the commander has authorized a delay.

DA Form 2408 Series

The 5988-E and 5990-E handle current activity. The DA Form 2408 series preserves the permanent history that follows the equipment for its entire service life.

The DA Form 2408-9, Equipment Control Record, is the master identification and transaction log. It tracks nomenclature, serial number, national stock number, manufacturer, year of manufacture, warranty period, and contract number, and it captures every transfer between organizations, shipment, and increment of usage.3Department of the Army. The DA Form 2408-5-1, Equipment Modification Record, logs every modification work order applied to a component: the MWO number and date, the title, the organization that performed the work, the labor hours, and the serial numbers of the affected components.4ArmyReal. Equipment Status Codes

The fault entries on the 5988-E drive one of three readiness designations, which then roll up into unit-level readiness reporting.2Aviation Assets. Army Regulation 750-1

  • Fully Mission Capable (FMC). Equipment is safe, mission-essential subsystems are installed and operating, and no faults exist that would appear in the “not FMC ready if” columns of the applicable PMCS tables.
  • Partially Mission Capable (PMC). Equipment can perform at least one but not all assigned missions because one or more subsystems are inoperable. PMC breaks down into PMC-Maintenance and PMC-Supply depending on whether the limit is a repair in progress or a parts shortage.
  • Not Mission Capable (NMC). Equipment cannot perform any combat mission. NMC splits the same way into NMC-Maintenance and NMC-Supply.

The maintenance-versus-supply split is what makes the data useful. A unit at 70% FMC because parts are on backorder needs a different answer than one at 70% because its maintainers are overwhelmed, and commanders use that split to decide whether to push for parts, people, or better planning.

When the NMC Clock Starts

For unscheduled maintenance, NMC-Maintenance time begins when the malfunction is discovered or at mission completion, whichever comes later. NMC-Supply time starts when a parts shortage halts work and the requisition remains unfilled one hour after the demand is initiated.5Department of Defense. DoD Instruction 3110.05 Those rules exist to keep units from gaming the numbers by delaying when the clock starts. Reporting happens at least monthly, and rates that fall below established goals trigger a formal problem identification and correction process.

PMCS Is Where the Documentation Starts

Preventive Maintenance Checks and Services are the front end of the whole system. Operators and crew members work through the equipment’s technical manual, which lists what to check, when to check it, and how to check it, and they perform the checks before, during, and after any movement or use.6U.S. Army. Before-operation checks catch problems while the equipment is parked. During-operation checks monitor gauges and performance under load. After-operation checks confirm nothing degraded during the mission and set the baseline for the next crew. Every deficiency at any stage has to be recorded on the appropriate form. Skipping the step, or downgrading a fault to keep an aircraft green on paper, defeats the purpose of the record and creates the hazard the paperwork was designed to catch.

Oil Analysis and Warranty Reporting

The Army Oil Analysis Program feeds into the same documentation stream. AOAP laboratories analyze oil and fluid samples from engines, gearboxes, and transmissions to detect internal wear before a component fails, then send a diagnostics report back to the owning unit with a recommendation on what needs attention.7U.S. Army. Warranty claims run through the same channel. Who Is Responsible

Compliance is not a shop problem. It runs from the operator to the commander, and a weak link at any level degrades the whole system.

Operators and crew members execute PMCS and record every deficiency honestly. Everything downstream depends on this. A pencil-whipped PMCS puts bad data into a system commanders rely on for deployment decisions.

Supervisors and platoon leaders verify that PMCS was actually done and that the recorded status matches what the equipment looks like. They spot-check forms, catch downgraded faults, and keep the move from fault discovery to maintenance request from stalling.

Maintenance warrant officers are the technical experts for data integrity. Commanders own the outcome. They set the conditions that make good maintenance possible by allocating time, prioritizing resources, and holding leaders accountable. Treat maintenance as something that happens after training, and the readiness numbers will show it.

How Compliance Gets Checked

Compliance with the pamphlet is not left to the honor system. The Command Maintenance Discipline Program provides the evaluation framework, and Command Maintenance Evaluation Teams conduct external inspections using a standardized checklist that covers maintenance management, shop operations, PMCS execution, AOAP, modification work orders, tool management, and related areas.10Army National Guard. The evaluation thresholds are concrete. A compliant rating for primary weapons systems and equipment requires at least 90% Fully Mission Capable across reportable items. General materiel requires 80% compliance on inspected equipment. Maintenance management programs must reach 90% compliance across all evaluated areas.10Army National Guard. On-the-spot training and mentorship are encouraged during the inspection, but a noncompliant rating is still noncompliant, and it moves quickly up a commander’s priority list.
