There is no document literally called the Cigna Formulary Exception Form. When your prescribing doctor needs Cigna to cover a drug that isn’t on your plan’s formulary, they submit the request either electronically through CoverMyMeds or on a Cigna Medication Prior Authorization form, and federal rules give Cigna 72 hours to decide a standard request and 24 hours to decide an urgent one.1eCFR. 45 CFR 156.122 – Prescription Drug Benefits
Which Form You Actually Need
Cigna handles two different exception situations, and they use different forms.
A coverage exception asks Cigna to pay for a drug that isn’t on the formulary at all. If approved, the drug counts as a covered essential health benefit, so any cost-sharing you pay goes toward your plan’s annual out-of-pocket maximum. These requests go on Cigna’s Medication Prior Authorization form, or electronically through CoverMyMeds.
A tier exception is different. The drug is already on your formulary, but it sits on a higher-cost tier and your doctor wants Cigna to charge you the lower preferred-brand copay. Tier exceptions use a separate Copay Reduction Request Form and go to a different fax line, (866) 249-1172, or by phone at (800) 558-9363.
Cigna prefers electronic submission for coverage exceptions. When You Qualify
Federal rules require health plans offering essential health benefits to let you, your representative, or your prescribing doctor request access to drugs not otherwise covered.1eCFR. 45 CFR 156.122 – Prescription Drug Benefits The working standard is medical necessity: your doctor has to show the non-formulary drug is the right treatment for your condition.
Most requests come up because of step therapy, where the plan requires you to try preferred drugs first. If those preferred options didn’t work, caused side effects, or weren’t appropriate for you, your doctor documents the failures and files the exception. What Has to Be on the Form
The clinical justification carries the request. A note saying you prefer the drug doesn’t work. What does: specific dates of prior medication trials, documented adverse reactions, lab results showing inadequate response, and a clear explanation of why the requested drug is expected to produce a better outcome.
The alternative medication history section is where most requests are won or lost. For each drug you tried before, your doctor should list the manufacturer, dates, duration, and what went wrong — side effects, lack of response, documented contraindication. If you haven’t tried alternatives at all, the form has a space for your doctor to explain why you can’t use them.
Make sure your Cigna ID number comes straight off your insurance card rather than from memory. A transposed digit routes the request to the wrong plan and delays everything. The urgency checkbox matters too: marking the request urgent triggers the 24-hour timeline, but your prescriber has to attest that waiting the standard period could seriously jeopardize your life, health, or ability to regain maximum function.
How to Submit and How Fast Cigna Must Respond
Three ways in, ranked by speed:
- Electronic through CoverMyMeds.com/epa/Cigna or an EHR integration. Usually generates immediate confirmation of receipt.
- Phone to Cigna’s pharmacy line at 1-800-882-4462. Providers with pharmacy-specific prior authorization questions can also use 1-800-266-6224.

Fax or electronic submission gives you a timestamped record of when Cigna received the request, which matters if a timeline dispute comes up later.
Once Cigna has the request, the clock is federal, not optional. A standard review has to be decided and communicated to you and your prescriber within 72 hours. An expedited review, which applies when your condition could seriously jeopardize your life, health, or ability to regain maximum function, or when you’re already taking a non-formulary drug as part of ongoing treatment, must be decided within 24 hours.5Cigna Healthcare. If approved, the authorization is entered into Cigna’s claims system and the pharmacy can fill the prescription with coverage applied. Your doctor’s office will usually let you know the moment they get confirmation.4Cigna Healthcare. How to Request an Exception / Approval
How Long an Approval Covers You
For a standard exception, federal rules require the plan to cover the drug for the full length of the prescription, including refills.1eCFR. 45 CFR 156.122 – Prescription Drug Benefits An expedited exception granted under urgent circumstances lasts for the duration of that urgent situation. Cigna’s internal coverage policy generally sets exception approvals at 12 months unless the specific drug criteria say otherwise.6Cigna Healthcare. When the approval period ends, your doctor has to submit a new request. Don’t wait until you’re down to a few pills. Build in several weeks of lead time before the current approval expires so review happens before your supply runs out.
If Cigna Denies the Request

For commercial Cigna plans, you generally have 180 days from the date on the denial to file an internal appeal. Use that window to add anything new your doctor can produce: updated labs, specialist notes, published studies supporting the drug for your condition, or a sharper account of why alternatives failed. Cigna must decide an internal appeal within 30 days if you haven’t yet received the drug, or 60 days if you’ve already paid out of pocket.8HealthCare.gov. Appealing a Health Plan Decision
For urgent situations, you can request an expedited internal appeal. Cigna has to respond as quickly as your condition requires and no later than four business days after receiving it. If the internal appeal is also denied, federal rules give you the right to an external review by an independent review organization that has no financial relationship with Cigna.9eCFR. 45 CFR 147.136 – Internal Claims and Appeals and External Review Processes The IRO does its own clinical evaluation and issues a binding decision. The denial letter from your internal appeal includes instructions for requesting it. This is the strongest tool available to you, because the IRO reviews the case from scratch rather than just checking whether Cigna followed its own procedures.
Why Requests Get Denied
Most denials come down to a handful of preventable problems:
- Incomplete forms. A missing Cigna ID, blank diagnosis field, or unsigned form can trigger an automatic denial before any clinician reviews the medical case.
- No documented alternative trials. If the alternative medication section is blank and nothing explains why you can’t try preferred drugs first, Cigna will almost certainly deny the request. Step therapy documentation is not optional.
- Vague clinical justification. “Patient needs this medication” doesn’t meet the bar. Specific dates, dosages, adverse reactions, and lab values carry the argument.
- Drug not recognized as appropriate for the diagnosis. Cigna cross-references the requested drug against its clinical guidelines for the condition on the form. If the two don’t line up, the request is denied on clinical grounds no matter how carefully the form was filled out.
If your denial was based on incomplete information rather than a clinical judgment, your doctor can often resubmit with the missing details instead of going through a formal appeal. Read the notice carefully — it should spell out exactly what was missing.
