The Charles Schwab name change form — officially the “Change Your Legal Name” form — is a downloadable PDF on schwab.com that updates the name on your brokerage, retirement, and Schwab Bank accounts after a marriage, divorce, or court-ordered name change. You fill it in, sign it, attach a legible copy of the legal document that proves the change, and send it to Schwab through the secure Message Center, by mail, or in person at a branch. Update Social Security First
Financial institutions check your name-and-SSN combination against SSA records. If Schwab’s systems can’t match your new name to your Social Security number, the update won’t go through cleanly, and any tax reporting that goes out under the mismatched name can trigger IRS backup withholding on your account.
File Form SS-5, the Application for a Social Security Card, at your local Social Security office. Name changes cannot be processed online. Bring an original or certified copy of the document proving the change — a marriage certificate, divorce decree, or court order — plus a current identity document such as a U.S. driver’s license or passport. Photocopies and notarized copies aren’t accepted. Wait at least 48 hours after your SSA visit before submitting to Schwab, so the SSA record has time to update.
What the Form Asks For
Download the PDF from schwab.com. You can find it by searching “change name” in the Forms and Applications section, or through your account dashboard under profile settings. Save it locally before filling it out.
The fields are straightforward:
- Current account name, exactly as it appears on your existing Schwab registration.
- New legal name, exactly as it appears on your supporting document.
- Every Schwab account number you want updated. Schwab account numbers are eight digits; a recent statement or your account dashboard will show them.
- Your Social Security number, which ties the accounts to your SSA record and to the 1099-B and 1099-DIV forms Schwab files with the IRS.
- A handwritten signature.
On a joint account, the form needs to specify which account holder is changing their name. Both holders do not need to sign unless both names are changing.
The Supporting Legal Document
Schwab needs one legal document linking your old name to your new one. Accepted options:
- A certified marriage certificate from the county or state vital records office.
- A divorce decree that specifies your restored or new legal name.
- A court-ordered name change bearing the official seal of the issuing court.
Send legible copies, not originals. Documents submitted by mail or uploaded digitally will not be returned. Blurry or cut-off copies are the most common reason Schwab flags a request as “Not in Good Order” and kicks it back.
Medallion Signature Guarantee
A standard name change on an existing account typically does not require a Medallion Signature Guarantee. Schwab accepts the completed form three ways.

Log in at schwab.com, open the Message Center, choose “Upload Document,” and attach the completed PDF along with a scan or clear photo of your legal document. After You Submit
Expect the update to take several business days once Schwab has a complete submission. Confirmation comes by email or secure message, depending on your contact preferences. Log in afterward and check that the new name appears on your profile and on every account you listed.
If Schwab flags the request as “Not in Good Order,” it’s usually one of three things: a blurry copy of the legal document, a missing signature, or an account number that doesn’t match their records. Confirm all three before sending.
Cards, Checks, and Linked Accounts
The change on your investment accounts does not automatically ripple through to a Schwab Bank Investor Checking account. Request a replacement debit card through the Accounts tab or by phone; your old card keeps working, but merchants that verify the name on file can push back. Also check any external bank accounts linked for ACH transfers. If those still carry your old name, transfers can be rejected or delayed once the Schwab side switches over.
Beneficiaries and Power of Attorney
A name change is a good moment to review beneficiary designations on your IRAs, brokerage accounts, and Transfer on Death registrations. Those designations pass assets directly based on what Schwab has on file, independent of any will. If the name change stems from a divorce, an unrevised beneficiary designation can send assets to a former spouse.
If someone holds power of attorney over your accounts, that arrangement may need updating too. The Tax Reason to Move Quickly
Schwab reports your dividends, interest, and capital gains to the IRS using the name and SSN on your account. If those don’t match SSA records, the IRS can send Schwab a CP2100 notice flagging the mismatch.9Internal Revenue Service. Backup Withholding
When that happens, Schwab is required to begin backup withholding at 24% on reportable payments — dividends, interest, capital gains proceeds — until the mismatch is resolved.10Internal Revenue Service. 2026 Publication 15 The withheld money goes to the IRS and is recoverable on your return, but the cash-flow hit and cleanup aren’t worth it. To stop backup withholding after a first notice, you provide Schwab with a corrected Form W-9 showing your new name.11Internal Revenue Service. Trust, Business, and Custodial Accounts
The individual name change form does not cover trust, business, or custodial accounts. Most trust accounts are titled with the trustee’s name, so a personal name change usually means the trust registration needs updating through a separate process.
