Centene Prior Authorization Form: Filing, Deadlines, and Appeals

A Centene prior authorization form is the request a provider submits to a Centene subsidiary — Ambetter, WellCare, Health Net, Superior HealthPlan, Peach State Health Plan, and others — to get approval for a service, procedure, or drug before delivering it. Each subsidiary publishes its own version of the form tailored to the state and line of business, so the first task is picking the right one; the second is filling it out completely enough that it is not kicked back. Starting January 1, 2026, Centene’s Medicaid and Medicare Advantage plans must issue standard decisions within seven calendar days and urgent decisions within 72 hours.1eCFR. 42 CFR 438.210

Confirm the Service Actually Needs Authorization

Before downloading anything, verify that the service requires prior authorization under the patient’s specific plan. Each Centene subsidiary runs an authorization lookup tool on its provider portal. Services that routinely require authorization include advanced diagnostic imaging (MRI, CT, PET), inpatient admissions, outpatient surgeries, specialty referrals, durable medical equipment, and high-cost or specialty medications. Gather Everything Before You Open the Form

Incomplete forms get rejected, and rejection restarts the decision clock. Centene subsidiary forms typically warn in bold that incomplete submissions will be sent back and that missing clinical records will delay the determination.3Superior HealthPlan. Patient and Provider Identifiers

You need the patient’s full legal name, date of birth, and the Centene subsidiary member ID from the insurance card. On the provider side: requesting physician name, NPI, TIN, office address, phone, and fax. If the rendering provider or facility is different from the requesting physician, include separate identifying information for both. These fields sit at the top of every Centene form, whether inpatient, outpatient, or pharmacy.

Diagnosis and Procedure Codes

The clinical core of the form is ICD-10-CM diagnosis codes that justify medical necessity, paired with CPT or HCPCS procedure codes that describe exactly what you are requesting.4Health Net. Clinical Documentation

Attach office notes, recent labs, imaging reports, and the treatment plan. The reviewer needs to see that less intensive alternatives were tried and either failed or are clinically inappropriate, not just that the patient carries a qualifying diagnosis.

Step Therapy Exceptions for Drugs

Centene’s plans often require step therapy, meaning the patient has to try a preferred, lower-cost drug before the plan approves the prescribed one. Filling Out the Form

Layouts vary by subsidiary, so pick the form that matches the patient’s state, line of business (Medicaid, Medicare Advantage, or Marketplace), and service type (inpatient, outpatient, or pharmacy). Using the wrong form routes the request to the wrong review team and triggers a rejection.

Enter identifiers in the top section. In the middle section, put ICD-10-CM codes in the diagnosis fields and CPT or HCPCS codes in the procedure fields. Do not swap them: automated systems reject forms when codes land in the wrong boxes. Check the inpatient or outpatient box. Most forms include a narrative section where you can summarize the clinical rationale in plain language, and that space is worth using when the codes alone do not tell the whole story. A short, concrete explanation of why this patient needs this service now gives the reviewer context codes cannot carry.

The requesting physician must sign and date the form, and expedited requests specifically require the requesting physician’s signature.3Superior HealthPlan. Submitting the Form

The fastest submission channel depends on whether you are requesting a medical service or a drug.

Pharmacy Requests

For prescription drugs, Centene prefers electronic submission through CoverMyMeds. The ePA process is HIPAA-compliant and can issue immediate approval for select drugs and plans, updating the patient’s record so claims adjudicate right away.7Centene Pharmacy Services. Medical Requests

For medical services, the primary channel is the subsidiary’s secure provider portal. Log in, go to Care Management or the Authorization section, and upload the completed form with all supporting documentation. The portal gives you immediate confirmation of receipt; save it. If the portal is unavailable, fax the completed form and clinical documentation to the department-specific fax number listed on the plan’s contact page. How Long Centene Has to Decide

Federal rules tightened the response windows effective January 1, 2026.

  • Medicaid managed care, standard: no more than seven calendar days after receipt, down from 14 for rating periods before 2026. The plan can extend by up to 14 additional days if the enrollee or provider asks, or if the plan needs more information and can show the delay benefits the enrollee.1eCFR. 42 CFR 438.210
  • Medicaid managed care, expedited: no more than 72 hours when a provider indicates, or the plan determines, that the standard timeframe could seriously jeopardize the enrollee’s life, health, or ability to regain maximum function.1eCFR. 42 CFR 438.210

Decisions come back through the portal, by fax, mail, or phone. A denial notice includes the specific reason and instructions for contesting it.

Emergency Care Is Carved Out

Do not delay emergency treatment to obtain authorization. Under EMTALA, hospitals cannot delay a medical screening exam or stabilizing treatment to ask about insurance or payment, and the prudent layperson standard — extended to Medicaid managed care by the Balanced Budget Act of 1997 and to individual and small-group plans by the ACA — bars insurers from denying emergency claims based on the final diagnosis if a reasonable person would have believed emergency care was needed.11ACEP. If the Request Is Denied

A denial is not final. Start with the fastest option and escalate from there.

Peer-to-Peer Review

The requesting physician has five business days from the denial notification to request a peer-to-peer review, which is a phone conversation with a Centene medical director who was not involved in the original decision. The provider should receive fax notification of peer-to-peer rights within one business day of the denial. On the call, the treating physician explains the clinical rationale directly, and the medical director can reverse the denial. Miss the five-day window and the only option left is a formal appeal. Internal Appeals

For Medicare Advantage plans, the first appeal is filed with the plan within 60 days of the denial notice, and the plan has 60 days to decide. A denial or non-response automatically moves the case to the Independent Review Entity, which also has 60 days. Further escalation runs to the Office of Medicare Hearings and Appeals, the Medicare Appeals Council, and federal district court, with claim-value thresholds along the way.13Medicare Interactive. External Review

After exhausting internal appeals, patients on ACA-compliant plans can request an independent external review in writing within four months of the final internal denial. An independent reviewer not affiliated with the insurer issues a binding decision the insurer must accept. Standard external reviews finish within 45 days; expedited reviews, for medically urgent cases, within 72 hours or less. The federal process administered by HHS is free to the patient, and other processes cap the fee at $25.14HealthCare.gov.
