The CDL self-certification form is a short state document where you tell your driver licensing agency which of four categories of commercial driving you do, so the agency knows whether to require a federal medical examiner’s certificate, a state medical certificate, or neither.1eCFR. 49 CFR 383.71 – Driver Application and Certification Procedures Filling it out is quick. Picking the right category, and keeping it current, is what actually protects your CDL.
Pick One of the Four Categories
The form turns on two questions: does your driving cross state lines, and is your operation exempt from federal medical qualification rules? Your answer places you in one of four boxes.2Federal Motor Carrier Safety Administration. Medical
Non-excepted interstate. You drive across state lines, or carry cargo or passengers that originated in another state, and you must hold a current federal medical examiner’s certificate. Most over-the-road truckers sit here.1eCFR. 49 CFR 383.71 – Driver Application and Certification Procedures
Excepted interstate. You drive across state lines but only in an operation federal rules exempt from medical certification. The main exemptions cover school bus operations, vehicles operated by federal, state, or local governments, fire trucks and rescue vehicles during emergencies, and transporting human corpses or sick and injured persons. Custom-harvesting operations, beekeeping, and certain farm vehicle roles also qualify.3eCFR. 49 CFR 390.3 – General Applicability4eCFR. 49 CFR 391.2 – General Exceptions
Non-excepted intrastate. You drive only within one state and are subject to that state’s own medical qualification rules. Local delivery drivers handling goods produced and sold within the same state usually fall here.1eCFR. 49 CFR 383.71 – Driver Application and Certification Procedures
Excepted intrastate. You drive only within your state in a role your state has determined does not require meeting its medical certification standard. What qualifies varies by state, and some exempt certain agricultural or utility drivers.5Federal Motor Carrier Safety Administration. How Do I Determine Which of the 4 Categories of Commercial Motor Vehicle (CMV) Operation I Should Self-Certify
The trickiest call is interstate versus intrastate. The test is whether your cargo or passengers have any connection to another state, not whether your truck crosses a line. Lumber picked up from a local mill and taken to a local job site is intrastate. That same lumber, shipped into your state from out of state with you running the last leg, is interstate commerce. When in doubt, non-excepted interstate is the safest pick: it requires the most paperwork but avoids driving in a category you don’t actually qualify for. Driving under the wrong self-certification can lead to suspension or revocation of your CDL.6Federal Motor Carrier Safety Administration. Medical
What to Have on Hand Before You Fill It Out
The form itself is typically one page. It asks for your full legal name as it appears on your license, date of birth, and your CDL or CLP number. Some states also ask for your Social Security number.
If you pick either non-excepted category, you also need to give your state a copy of your medical examiner’s certificate, Form MCSA-5876.7Federal Motor Carrier Safety Administration. Medical Examiners Certificate (MEC), Form MCSA-5876 That’s the card your DOT medical examiner gives you after your physical, showing the examiner’s name, their National Registry number, and the certificate’s issue and expiration dates.8Federal Motor Carrier Safety Administration. Medical Examiners Certificate Form MCSA-5876 As of June 23, 2025, medical examiners transmit certificate information electronically to FMCSA, which forwards it to your state’s CDLIS driver record.9eCFR. 49 CFR 383.73 – State Procedures Even so, some states still want you to bring or mail a copy, so check with your agency.
If you pick an excepted category, no medical examiner’s certificate is required and the self-certification form alone satisfies your obligation.1eCFR. 49 CFR 383.71 – Driver Application and Certification Procedures
Where to Get the Form and How to File It
Your state’s DMV, Department of Transportation, or equivalent licensing agency issues the form. Most states post it as a downloadable PDF and also offer a driver portal for online filing. The exact name and number varies by state; look for “CDL self-certification” or “medical certification” on the agency’s CDL page. FMCSA keeps a directory of state licensing agency contacts if you need help finding the right office.10Federal Motor Carrier Safety Administration. States
Submission methods generally come down to three:
- Online through a driver portal, where you can usually select your category and upload the medical certificate in one session. This is the fastest option and gives immediate confirmation.
- In person at a licensing office with the completed form and your medical certificate, walking out with an updated record.
- By mail or fax, where some states still accept paper submissions. Keep a copy of everything you send and use certified mail or save the fax confirmation as proof.
Whichever way you file, verify afterward that your record has been updated. An outdated or missing self-certification shows as “not certified” during traffic stops and compliance reviews.
When You Need to File a New One
Three situations require a new or updated filing.
The most common is your medical examiner’s certificate expiring. For most drivers, that’s every 24 months.11eCFR. 49 CFR 391.45 – Persons Who Must Be Medically Examined and Certified Your examiner can issue a shorter certificate to monitor a condition like high blood pressure, in which case you may need a new exam annually or more often.12Federal Motor Carrier Safety Administration. DOT Medical Exam and Commercial Motor Vehicle Certification Get the new certificate to your state before the old one expires.6Federal Motor Carrier Safety Administration. Medical
The second is a change in your work. If you move from local intrastate deliveries to cross-country hauling, your category shifts from intrastate to interstate, and you file an updated form before the new work starts. Driving under the wrong certification type can trigger suspension or revocation.6Federal Motor Carrier Safety Administration. Medical
The third is switching between excepted and non-excepted status. A driver leaving over-the-road trucking to drive a school bus goes from non-excepted interstate to excepted interstate and no longer needs a federal medical certificate. File the updated form promptly so the state record matches your actual operation.
What a Lapse Costs You
Within 10 calendar days of your medical certificate expiring, your state must update your CDLIS record to show a medical status of “not-certified.”9eCFR. 49 CFR 383.73 – State Procedures The state then notifies you that your CDL will be downgraded unless you fix it. The downgrade must be completed within 60 days of the status change.10Federal Motor Carrier Safety Administration. States
A downgrade reverts your license to a standard non-commercial class. You cannot legally drive any vehicle that requires a CDL until you fix the underlying problem, and inspectors running your record at a traffic stop will see the “not-certified” status immediately.
To avoid or reverse a downgrade, you can:
- Get a new medical exam and submit the current certificate to your state before the downgrade is finalized.
- Submit a medical variance, such as an exemption, waiver, or Skill Performance Evaluation certificate, if one applies to your situation.
- Change your self-certification to an excepted category, but only if your actual operations genuinely fall under an exemption. You cannot reclassify yourself to dodge the medical requirement.
Reinstatement after a completed downgrade varies by state. Some charge a reinstatement fee on top of requiring a current medical certificate and updated self-certification. For most drivers, the bigger cost is the time off the road.
If You Hold a Medical Variance
Drivers who don’t meet every federal physical qualification standard aren’t necessarily locked out. FMCSA runs the Skill Performance Evaluation program for drivers with certain limb impairments,13Federal Motor Carrier Safety Administration. Skill Performance Evaluation Certificate Program an alternative vision standard applied by your medical examiner using Form MCSA-5871,14Federal Motor Carrier Safety Administration. General Vision Exemption Package and a federal hearing exemption process that requires a Federal Register notice and 30-day public comment period before a final decision.15Federal Motor Carrier Safety Administration. Federal Hearing Exemption Application If you hold a variance, your state records it on your CDLIS record alongside your medical certificate. Keep a copy of the exemption letter or SPE certificate with you while on duty.9eCFR. 49 CFR 383.73 – State Procedures
Canadian and Mexican License Holders
The self-certification requirement is a U.S. state process for CDL and CLP holders. The United States has reciprocal CDL agreements with Canada and Mexico only.16Federal Motor Carrier Safety Administration. Commercial and Non-Commercial Driver License Requirements Canadian commercial license holders may operate in the U.S. under their home license, and those restricted for medical reasons may carry a “code W” restriction, though not every province has implemented it. For Mexican drivers, the U.S. recognizes only the Licencia Federal de Conductor issued by Mexico’s federal transportation authority; state-issued Mexican licenses are not accepted for commercial vehicle operation in the United States.
