Can You Use HSA for Wegovy? Eligibility, Documentation, and Payment

Yes, you can use an HSA for Wegovy when a doctor prescribes it to treat a diagnosed medical condition such as obesity, overweight with a related health problem, cardiovascular disease, or metabolic liver disease. The IRS treats prescription drugs that treat or prevent disease as qualified medical expenses, and it has specifically recognized obesity as a disease whose treatment counts.1Internal Revenue Service. Why the IRS Allows It

That line is what makes the diagnosis so important with a drug like Wegovy. In Revenue Ruling 2002-19, the IRS held that amounts paid for weight-loss treatment qualify as deductible medical expenses when a physician has diagnosed a specific disease, and it named obesity as one such disease.1Internal Revenue Service. Prescribed only to help someone slim down for appearance, it is not, and pulling HSA money for it would be a non-qualified distribution.

Which Diagnoses Qualify

The FDA has approved Wegovy for several medical uses, and any of them supports treating the expense as HSA-eligible:

  • Weight reduction in adults and patients aged 12 and older with obesity.
  • Weight reduction in adults who are overweight and have at least one weight-related condition, such as type 2 diabetes or hypertension.
  • Reducing the risk of heart attack, stroke, or cardiovascular death in adults with established cardiovascular disease and either obesity or overweight.

The IRS ruling on obesity treatment does not limit qualifying diagnoses to obesity itself. Hypertension, type 2 diabetes, and cardiovascular disease all provide a valid medical basis when your provider identifies the condition and prescribes Wegovy to address it.1Internal Revenue Service. Letter of Medical Necessity

Before you spend HSA funds on Wegovy, ask your prescriber for a Letter of Medical Necessity. This is your proof, on paper, that the drug treats a diagnosed disease rather than a cosmetic concern. A thorough letter includes your name, the diagnosed condition (often with the ICD-10-CM code, such as E66.9 for obesity), the prescribed medication, and the physician’s dated signature. Some HSA administrators also want the provider’s National Provider Identifier.

Most administrators treat the letter as valid for 12 months from its date. If treatment continues past a year, get a renewed letter. Asking your provider to note a treatment duration up front avoids surprises about when the next letter is due.

Pharmacy Receipts

Save a detailed receipt every time you fill the prescription. At minimum, the receipt should show:

  • The date the prescription was filled.
  • The drug name (Wegovy or semaglutide).
  • The patient’s name.
  • The amount you actually paid, after any insurance or discount.

A missing drug name or date is the most common reason a claim gets bounced. Keep both digital and paper copies of receipts and your Letter of Medical Necessity. You may need them years later if the IRS questions a return or your HSA administrator runs a compliance check.

Debit Card at the Pharmacy

The simplest route is your HSA debit card at the pharmacy counter. The transaction pulls directly from your tax-advantaged balance, and pharmacies with integrated systems often auto-substantiate the charge as a qualified medical expense based on the merchant category and prescription data.5Internal Revenue Service. Manual Reimbursement

If the pharmacy won’t take your HSA card, pay another way and reimburse yourself later. Log into your HSA administrator’s portal, upload the pharmacy receipt and Letter of Medical Necessity, and enter the exact amount you paid. Reimbursement generally lands in your linked bank account. There is no deadline: you can pay out of pocket today and reimburse yourself months or years later, as long as the HSA existed before the expense was incurred.5Internal Revenue Service. When You Use a Manufacturer Discount

When you use a coupon or savings card, your HSA-eligible amount is what you actually pay after the discount, not the list price. If a card drops your fill from $349 to $249, only $249 can come from the HSA for that fill.

What Happens If the Expense Doesn’t Qualify

Pull HSA funds for a purchase that isn’t medical care, or one you can’t substantiate as medically necessary, and the amount is added to your taxable income and hit with an additional 20 percent tax.7Internal Revenue Service. Two exceptions matter. Once you reach age 65, the 20 percent additional tax no longer applies; you still owe income tax on a non-qualified withdrawal, but the penalty disappears. And if you catch the mistake before your tax-filing deadline for that year (generally April 15 of the following year, without extensions), you can return the money to your HSA as a mistaken distribution and avoid both the penalty and the income tax. If You Don’t Have an HSA

Two other tax-advantaged accounts can cover Wegovy under the same medical-expense rules. A standard health flexible spending account works for prescribed weight-loss drugs when a diagnosis is documented. A health reimbursement arrangement funded by your employer can also reimburse the cost, though coverage depends on how the plan is written.

A limited-purpose FSA won’t work: it is restricted to dental and vision expenses. If you have both an HSA and a limited-purpose FSA, the limited-purpose account can’t be used for Wegovy. Dependent care FSAs cover childcare, not medication.

One practical difference: FSA funds generally must be used within the plan year (plus any grace period or carryover your employer permits), while HSA funds roll over indefinitely. If Wegovy is likely to be a long-term prescription, the HSA gives you more room to accumulate and spend on your own schedule.
