Can You Get Passport Forms at the Post Office?

You can no longer count on grabbing a blank passport form at the post office counter. The U.S. Postal Service and the State Department now expect you to fill out Form DS-11 online using the State Department’s Form Filler, print the completed form on single-sided paper, and bring it with you to your appointment.¹ Some locations may still have paper copies tucked behind the counter, but relying on that is the slow path.

Where to Get the Form

Every passport form — DS-11 for first-time applicants, DS-82 for mail renewals, DS-3053 for a parent’s notarized consent, and the rest — lives on the State Department’s website at travel.state.gov.¹ The Form Filler tool lets you type your information directly into DS-11 and then print the finished document. Both USPS and the State Department recommend this method because it cuts down on the errors that cause processing delays. One rule catches people out: do not sign the printed form at home. A postal employee has to witness your signature in person, and a pre-signed DS-11 is invalid. If you sign early, you print again.²

Which Post Offices Accept Passport Applications

Not every post office is a passport acceptance facility. The ones that are keep separate — and often narrower — hours for passport work. To find a participating branch, use the State Department’s Passport Acceptance Facility locator at iafdb.travel.state.gov, which lets you search by ZIP code or city and filter for offices that also take passport photos on site.³

At the appointment, a postal employee acts as the acceptance agent. They check your identity, put you under oath, watch you sign the DS-11, and mail the whole package to the State Department.⁴

Booking an Appointment

Passport service at the post office is by appointment. You can schedule one through the USPS Retail Customer Appointment Scheduler online, at a self-service kiosk in a post office lobby, or at the retail counter.⁴ During peak season, roughly January through summer, slots fill weeks out, so book early.

What to Bring So the Trip Isn’t Wasted

The acceptance agent cannot process a partial application. If you’re missing a document, you go home and rebook.

Proof of U.S. Citizenship

Bring an original or certified copy of one of the following: a U.S. birth certificate issued by the city, county, or state (hospital certificates don’t count), a previously issued undamaged U.S. passport, a Consular Report of Birth Abroad, or a Certificate of Naturalization or Citizenship. Digital or electronic birth certificates are not accepted; the document has to be physical, with an official seal or stamp.²

Photo ID and Photocopies

Bring a physical, government-issued photo ID such as a driver’s license or state ID card, plus a photocopy of the front and back. You also need a photocopy of your citizenship document, front and back. Some post offices have a copier available; many don’t.²

Passport Photo

You need one 2×2 inch color photo taken within the last six months, with a white or off-white background, no shadows, and even lighting. Glasses, including prescription eyeglasses, must be off unless you have a signed doctor’s note explaining a medical reason to wear them.⁵ Many passport-accepting post offices take the photo on site for $15.⁴ Pharmacies, shipping stores, and warehouse clubs often do it for less.

Fees

You pay two separate fees at the appointment, to two separate places. Mixing them up is a common frustration.

The application fee, made out to “U.S. Department of State,” is $130 for an adult passport book, $30 for a passport card, or $160 for both. For a child under 16, the book is $100. Payment must be by personal check, certified check, cashier’s check, traveler’s check, or money order.
The acceptance facility fee is $35, regardless of age or document type. This goes to the post office. Payment methods vary by location, so ask ahead.

Both fees are non-refundable, even if the passport is not ultimately issued. Expedited processing adds $60, and 1-to-3-day delivery of the finished book adds $22.05.⁶

Renewals Don’t Belong at the Post Office

If you qualify to renew with Form DS-82, do not bring it to a post office counter. Acceptance facilities won’t process DS-82 applications and should not charge you the $35 acceptance fee for a renewal.⁷ Renewals go by mail to a National Passport Processing Center using a trackable service (not FedEx, UPS, or DHL, since the centers use P.O. Box addresses), or through online renewal at opr.travel.state.gov if you meet the eligibility rules.⁸

Children Under 16

A child’s passport always uses Form DS-11 and always requires an in-person visit to an acceptance facility. Both parents or legal guardians generally need to appear at the appointment with the child.⁹

If one parent can’t come, that parent must sign a notarized Statement of Consent (Form DS-3053) and provide a photocopy of the ID shown to the notary. A parent with sole legal custody can apply alone but must bring supporting documentation, such as a court custody order, a death certificate for the other parent, or a birth certificate listing only one parent.⁹ The total is $135: the $100 book fee plus the $35 acceptance fee.⁶

Other Places That Accept the Same Form

Post offices are the most common acceptance facilities, but not the only ones. Many county clerk offices and public libraries also accept DS-11 applications and do the same witnessing job. The State Department’s locator at iafdb.travel.state.gov covers all facility types, and every one of them requires an appointment.² Wherever you apply, the form itself comes from travel.state.gov — that’s the fastest starting point whether or not your local branch has a paper copy on hand.
