Army PMCS Form (DA 2404): Status Symbols, Signatures, and GCSS-Army

To fill out DA Form 2404, the Army’s Equipment Inspection and Maintenance Worksheet, complete the header identifying the equipment and the technical manual, then use the body table to log each fault against its TM item number with the correct status symbol, a clear description of the deficiency, and the corrective action taken. Sign it, get the maintenance supervisor’s signature, and turn it in to maintenance control. Army Regulation 750-1 authorizes the 2404 alongside DA Form 5988-E as the record for PMCS results and acceptance inspections, though current guidance is to record maintenance in GCSS-Army and use the paper 2404 only when the system is unavailable.1Army.mil. Start With the Right Blank

Pull a blank from the Army Publishing Directorate at armypubs.army.mil. The current edition is dated 1 February 2011.2Army.mil. Leader’s Guide to Maintenance and Services Older 1979-era copies still circulate in some motor pools. Check the revision date before you start writing.

The Header Blocks

The top of the form ties the whole inspection to a specific piece of equipment and a specific published standard. If the serial number is wrong or the TM reference is outdated, everything below it is attached to the wrong asset or the wrong checklist.

Copy equipment nomenclature, model, National Stock Number, and registration or serial number from the equipment’s data plate or property record. There is also a field for the responsible organization; enter the unit designation the way it reads on property records.3GovTribe. • Blocks 4a–4d capture usage: miles, hours, rounds fired, and hot starts. Put an “M” before a reading in miles or a “K” before kilometers. Leave a block blank if that measurement doesn’t apply, the equipment has no such meter, or the form covers more than one item.
  • Block 5 is the date. Use the day you actually perform the inspection, not the day you turn the form in.
  • Block 6 is the type of inspection or service: monthly, quarterly, semiannual, lubrication, and so on. If you’re doing more than one at once, list the symbols together, such as “L/S.”
  • Block 7 is the TM number and date. If the manual has changes, write “W/C” and the latest change number after the TM number, but keep the basic manual’s date. When two TMs cover the item, use the second TM block for the second reference.

The Body Columns

The body is a five-column table. Each row is one item you inspected from the PMCS table in the applicable Technical Manual.3GovTribe. PWS Attachment 4 ARL Modified DA Form 2404 (SEMI-ANNUAL PM)

  • Column a: the TM item number that matches what you checked. This ties your finding to a published standard anyone can look up.5Waru.edu. DA Form 2404 – Equipment Inspection and Maintenance Worksheet
  • Column b: the status symbol showing the equipment’s condition for that item (see below).
  • Column c: the deficiency or shortcoming, described precisely enough for a mechanic who has never seen the equipment to know what’s wrong. “Leaking” is not enough. “Hydraulic fluid leak at left front brake line fitting, roughly one drip per minute” gives someone something to work with.
  • Column d: the corrective action, meaning the repair performed or the parts ordered. If the fault is beyond your level, note that it needs higher-level maintenance support.
  • Column e: the initials and date of whoever completes the corrective action, confirming the fix.3GovTribe. PWS Attachment 4 ARL Modified DA Form 2404 (SEMI-ANNUAL PM)

The form also has a field for manhours spent on each task.5Waru.edu. DA Form 2404 – Equipment Inspection and Maintenance Worksheet Enter it honestly. Labor data feeds how the unit plans future maintenance windows and staffs them.

The Four Status Symbols

Column b uses four standard symbols. Getting this one right is what tells the next person whether the equipment is safe to operate.

  • X means inoperable. The fault makes the equipment unable to perform its primary mission or unsafe to run. The asset is deadlined and no one uses it until the repair is verified by a maintenance supervisor.6GovTribe. PWS Attachment 3 ARL Modified DA Form 2404 (ANNUAL PM)
  • A circled X means operable with limitations. There is a deficiency, but the equipment can still run under specific restrictions set by the unit commander or a qualified maintenance officer, who also decides how long it can continue before the fault must be fixed.
  • A horizontal dash means a scheduled inspection or service is past due. The equipment’s actual condition is unknown.
  • A diagonal slash means a minor material deficiency that affects efficiency but does not prevent safe operation or mission accomplishment.

When you’re not sure, use the more restrictive symbol. A supervisor can always downgrade a status after looking at the equipment. Upgrading a status that was too lenient after something breaks is a much worse conversation.

Signatures and Turn-In

When you finish the inspection, sign Block 8a using first name, middle initial, last name, and rank. Block 8b is generally left blank unless local policy calls for a time entry.4Tpub. Turn the signed hard copy in to your unit’s maintenance control section. Faults beyond unit-level repair go, along with the 2404, to the supporting maintenance facility.1Army.mil. Army Regulation 750-1 – Army Materiel Maintenance Policy Completed forms end up in the equipment’s historical record folder, and when the equipment transfers between units the record copies travel with it.

After the Outage: Get It Into GCSS-Army

Paper 2404s are meant to bridge gaps in GCSS-Army connectivity, not to run as a parallel filing system. Once the system is back up, any faults you logged on paper have to be entered into GCSS-Army so the digital record is complete.2Army.mil. Leader’s Guide to Maintenance and Services Units that have moved fully to digital maintenance tracking use DA Form 5988-E as the electronic counterpart alongside GCSS-Army.

Before You Sign: Article 107 Applies

A DA Form 2404 is an official military document. Marking equipment as serviceable when it has a known deadline fault, fabricating inspection results, or signing off on repairs that were never done exposes the person responsible to prosecution under Article 107 of the Uniform Code of Military Justice for a false official statement.7Office of the Law Revision Counsel. 10 USC 907 Art 107 – False Official Statements; False Swearing

To sustain a charge, the government must show that the person signed a false official document or made a false official statement, knew it was false, and intended to deceive. A statement is “official” when it has a clear and direct relationship to the speaker’s military duties or is made to someone carrying out a military function.8The United States Court of Appeals for the Armed Forces. Core Criminal Law Subjects – Crimes – Article 107 – False Official Statements A maintenance worksheet signed in a motor pool during assigned PMCS clears that bar. Punishment is set by court-martial and can include confinement, reduction in rank, and a punitive discharge. Beyond that, falsified records mean equipment fails when it’s needed, and the paper trail leads back to whoever signed the lie.
