Wipes are FSA eligible when they serve a medical purpose rather than a general hygiene one. Medicated wipes with an active drug ingredient — think hemorrhoid wipes, acne pads, and antiseptic wipes for wound care — can usually be purchased directly with an FSA debit card. Standard wipes used for everyday cleaning don’t qualify unless a doctor certifies they are medically necessary for a diagnosed condition.
The rule comes from federal tax law. To count as a qualified medical expense, a product must treat, diagnose, prevent, or manage a disease, or affect a structure or function of the body.1Office of the Law Revision Counsel. 26 USC 213 – Medical, Dental, Etc., Expenses Items used mainly for personal hygiene, comfort, or cosmetic care fall outside that definition. The IRS uses toothbrushes and toothpaste as its illustration: helpful for health, but ordinary personal-care items rather than treatments.2Internal Revenue Service. Wipes You Can Buy Directly
Since 2020, over-the-counter medicines and drugs have been permanently eligible for FSA reimbursement without a prescription. That change matters here because a wipe with an active drug ingredient listed on a Drug Facts label now qualifies for direct purchase, no paperwork required. These products typically process automatically when you swipe an FSA debit card.
- Hemorrhoid wipes containing witch hazel, lidocaine, or similar active ingredients sold specifically for hemorrhoid relief.
- Acne treatment pads saturated with salicylic acid, benzoyl peroxide, or comparable acne medications.
- Medicated wipes for chronic skin conditions like eczema or psoriasis, formulated with active anti-inflammatory or therapeutic ingredients.
- First-aid antiseptic wipes containing agents like benzalkonium chloride, intended for cleaning wounds or preventing infection.
Hand sanitizer and hand sanitizing wipes purchased to prevent the spread of COVID-19 also qualify under the IRS personal protective equipment guidance.2Internal Revenue Service. Wipes That Qualify With a Letter of Medical Necessity
Some wipes that would otherwise be considered personal care can become eligible if a licensed healthcare provider writes a Letter of Medical Necessity (LMN). The letter links the product to a diagnosed condition and turns an ineligible purchase into a reimbursable medical expense.
- Baby wipes used for wound or skin care. Standard baby wipes are not eligible on their own, but a doctor may certify they are needed to manage severe diaper dermatitis or a wound-care regimen.
- Saline wipes for medical treatment. If saline-moistened wipes are prescribed for ongoing wound care or post-surgical cleaning, an LMN can make them reimbursable.
- Adult incontinence wipes. These often qualify for reimbursement, though some plan administrators require an LMN along with a detailed receipt. Check your plan’s documentation rules.3FSAFEDS. Eligible Health Care FSA (HC FSA) Expenses
A valid LMN must be completed by a licensed practitioner. It should describe the diagnosed condition, name the specific product being recommended, state the expected duration of treatment, and confirm that the product is not for general health or cosmetic purposes.4FSAFEDS. Letter of Medical Necessity Form For chronic conditions, the provider can list “lifetime” as the duration rather than a set end date. Some plan administrators have their own template, so pull it from your provider’s portal before the appointment.
Wipes That Aren’t Eligible
Products used for routine hygiene, cleaning, or cosmetic care don’t meet the IRS medical-expense standard, and buying them at a pharmacy doesn’t change that. Your FSA debit card should decline them at checkout.
- Standard baby wipes used for ordinary diaper changes are personal-care items, not medical supplies.
- Makeup remover wipes serve a cosmetic purpose only.
- Facial cleansing cloths are personal care unless they contain an active acne medication with a Drug Facts label.
- Household disinfectant wipes are household supplies, even though they kill germs.2Internal Revenue Service. Publication 502 (2025), Medical and Dental Expenses
- Breast pump or bottle cleaning wipes are generally not covered, even though the breast pump itself is eligible.
The dividing line is whether the wipe treats or prevents a specific condition or just keeps you or your surroundings clean. Without an active drug ingredient and without an LMN, a wipe almost certainly falls on the ineligible side.
How the Purchase Works at Checkout
When you use an FSA debit card, an Inventory Information Approval System (IIAS) checks each item at the register against a master list of eligible products. The system charges your FSA card only for qualifying items and rejects the rest in the same transaction.5Special Interest Group for IIAS Standards (SIGIS). IIAS Certification Retailers that accept FSA cards but don’t specialize in healthcare are required to run an IIAS, so accidentally buying the wrong item with your card isn’t much of a risk.
The master list is maintained by the Special Interest Group for IIAS Standards (SIGIS).6Special Interest Group for IIAS Standards (SIGIS). Eligible Product List Overview If a medicated wipe you expect to be eligible gets declined, it may not yet be on the list. You can pay out of pocket and submit a manual claim for reimbursement through your plan administrator’s website or mobile app. Most claims process within a few business days, though timing depends on the administrator.7FSAFEDS. Records to Keep
Save an itemized receipt for any wipes bought with FSA funds. It should show the date, the store name, and the specific product. A credit card statement listing only a total isn’t enough. If an LMN was required, file a copy of it with the receipt.
The IRS generally expects you to keep records supporting tax-related items for at least three years from the date you file your return.8Internal Revenue Service. How Long Should I Keep Records FSA contributions reduce your taxable income, so your receipts and any LMN documents fall under that rule. Digital copies in a cloud folder work as well as paper.
