Annual SLT Remuneration Form: Filing, Timeline, and Taxes

To collect the $300 annual payment, the SLT remuneration form has to be completed with your service hours and identifying information, submitted to your SLT chairperson along with a signed IRS Form W-9, and backed by proof that you served at least 30 hours during the school year and attended the required Comprehensive Educational Plan and budget training.1New York City Public Schools. Regulation of the Chancellor A-655 The rest comes down to keeping clean records and getting the paperwork in before the school year closes out.

The Two Requirements Behind the Payment

Chancellor’s Regulation A-655 sets two conditions for the full $300:1New York City Public Schools. Regulation of the Chancellor A-655

  • Thirty hours of SLT service during the school year, including regular team meetings, subcommittee sessions, CEP development work, and related activities.
  • Attendance at the DOE’s annual training on the Comprehensive Educational Plan and school budget issues.

If you attended the training but did not reach 30 hours, you can still request a pro-rata share of the $300 based on the hours you actually served.1New York City Public Schools. Regulation of the Chancellor A-655 Skip the training, though, and you are not eligible for any payment no matter how many hours you logged.

What to Gather Before You Fill It Out

Pull these together before you sit down with the form:

  • Your attendance and hour logs. Keep a running record of every SLT meeting, subcommittee session, and related activity you attended, with the date, start time, end time, and a brief note on what took place. Detailed logs prevent disputes and make the form itself quick to complete.
  • Proof of the mandatory training. Save any certificate, sign-in sheet, or confirmation email from the DOE session. If nothing was issued, ask your SLT chairperson to confirm your attendance in writing.
  • A completed IRS Form W-9. The DOE needs your taxpayer identification number before it can release payment. Without a W-9 on file, the payer may be required to withhold 24 percent of your payment as federal backup withholding.2Internal Revenue Service. • Your full legal name and current mailing address. The name on the remuneration form should match your W-9 exactly. Non-employee members typically get paid by mail, so the address matters.

Filling Out the Form

Blank copies of the remuneration form come from the school principal or SLT chairperson. The form is short. Enter your identifying information: name, address, school name, and the school year. Then transfer the totals from your hour log.

If you logged 30 or more hours and attended the training, you are claiming the full $300. If you are requesting a pro-rata payment, enter your actual hour total and make clear on the form that you are asking for the proportional amount.

Check the name against your W-9 before you sign. A mismatch between the two documents is one of the most common reasons a payment stalls. Attach the W-9 and your supporting hour documentation to the form.

Accepting or Waiving the Payment

Every SLT member gets a personal choice: accept the $300 or waive it and donate the funds back to the school.1New York City Public Schools. Regulation of the Chancellor A-655 The decision is individual. Team bylaws cannot pressure you toward either option or impose a blanket policy that everyone donate. Mark the appropriate box on the form.

Where and When to Submit

Give the completed package, meaning the form, your W-9, and your supporting logs, to your SLT chairperson. Under A-655, each member is responsible for making sure the records documenting their hours are submitted to the chairperson for processing.1New York City Public Schools. Regulation of the Chancellor A-655 The chairperson or school financial secretary then reviews the package, confirms the hours meet the 30-hour minimum (or calculates the pro-rata amount), and initiates payment through the school’s administrative systems.

Turn your paperwork in as soon as your service hours are done for the year. Schools generally process these forms toward the end of the school year, and a late submission can push your payment into the next fiscal year or run into staff turnover over the summer.

Payment Timeline and Follow-Up

Most payments go out after the school year ends, once hours have been verified and the numbers have been entered into DOE systems. DOE employees usually receive the stipend through their existing payroll setup. Parent members and other non-employee participants typically get a separate check.

If several weeks pass after submission with no payment or confirmation, follow up with the school’s financial secretary or your SLT chairperson. They can check the status in the school’s financial system and flag missing signatures, an incomplete W-9, or data-entry errors. Catching problems before the fiscal year closes saves real time.

Taxes on the $300

The stipend is taxable income. The IRS treats payments made in exchange for services as gross income even when the amount is small, and you should report it on your federal tax return for the year you receive it.

For the 2026 tax year, the Form 1099-NEC reporting threshold rose from $600 to $2,000.3Internal Revenue Service. General Instructions for Certain Information Returns Because the SLT payment is $300, the DOE is generally not required to issue a 1099-NEC. Your obligation to report the income is the same either way.

Whether the payment triggers self-employment tax depends on how it is classified. If it is reported as non-employee compensation, Social Security and Medicare taxes may apply, though on $300 the amount is small. DOE employees who receive the stipend through payroll typically have applicable taxes withheld automatically.

Records to Keep

Hold onto your attendance logs, a copy of the completed remuneration form, and your W-9 for at least three years after payment. That covers the standard IRS audit window. If a dispute or audit begins before the three years are up, keep the records until it is fully resolved.
