What the Form Is

The specific fields are not publicly documented.
One quick clarification, because the form numbers get mixed up. OSI Form 61 is not the document used to record sworn statements during Air Force investigations. That is AF Form 1168 (Statement of Suspect/Witness/Complainant), which carries rights advisements, narrative blocks, and witness signatures. OSI Form 61 is a career-application questionnaire and nothing more.
Who Uses It

- ROTC cadets, evaluated through an annual application process before completing their degree.
- USAFA cadets, also evaluated annually before commissioning; interested cadets contact OSI Detachment 439 at the Air Force Academy.
- OTS civilians and active-duty members. Civilians coordinate through an Air Force recruiting office; active-duty members coordinate through their base Education Office. Both must list “71S — Special Agent” as their number-one career field choice.
- Crossflow officers (captains and below already commissioned in another career field) who want to reclassify into 71S. They must first secure tentative release from their current career field manager.

How to Get the Form
OSI Form 61 is not available for public download. OSI-specific forms are distributed through AFOSI channels, so the reliable way to obtain it is to contact a geographically nearby AFOSI unit. The Two Forms That Travel With It

- OSI Form 65 (Writing Sample): a written product demonstrating analytical and communication ability.
- OSI Form 151 (Unit Leadership Recommendation): a recommendation from AFOSI unit leadership, typically completed after your visit or interview with a local detachment.
Cadets and active-duty applicants must also have their commander furnish one of the three required letters of recommendation.
Where to Submit It
The submission channel depends on your applicant category. OTS applicants coordinate through their base Education Office (active duty) or a Recruiting Office (civilians). Board schedules follow Air Force Recruiting Service deadlines, and 71S selections are announced alongside AFRS program acceptance results.2Air Force Office of Special Investigations. USAFA cadets coordinate directly through OSI Detachment 439 at 719-333-3305. ROTC cadets should refer to the annual ROTC-specific accession guidance published by AFOSI.
Do not wait until the deadline. The local AFOSI detachment visit that feeds into OSI Form 151 takes time to schedule, and the detachment needs lead time to evaluate you and complete the recommendation.
What Happens After You Submit
A board of senior special agents and professional staff reviews each package. If you are selected, AFOSI initiates an Agent Suitability Investigation, which includes paperwork for a Top Secret security clearance.2Air Force Office of Special Investigations.
